Obsessive Compulsive Behaviour - How To Deal With It?

Obsessive compulsive behaviour is a form of anxiety disorder in which unreasonable thoughts and fears, which are known as obsessions lead one to do repetitive compulsive behaviour. Root causes of obsessive compulsiveness are complex and often deep seated. The underlying emotional states may include the fear of unknown, not being in control, negative outcomes, failure, rejection, shame, annihilation or embarrassment. Many neurobiological factors have been implicated including alterations in some neural circuits and abnormalities of neurochemicals.

One can manage mild obsessions and compulsions easily, whereas, severe obsessive compulsiveness require support of mental health professionals.

The treatment for obsessive compulsive behaviour depends on the severity, which is reflected by how much the condition is affecting the daily life of the person. There are two main treatments. First is a set of psychotherapy techniques. The most well studied of these techniques is cognitive behavioural therapy, which involves several cognitive techniques as well as behavioural techniques like graded exposure and response prevention.

The cognitive behaviour therapy emboldens one to face one's fear and let the obsessive thoughts occur without neutralising them with compulsions. During the sessions, one works with therapist to break down the problems into their separate parts like physical feelings, thoughts and actions. The therapist encourages you to face your fear and let the obsessive thoughts occur without acting on them. There are several simple cognitive techniques that one can use to control obsessive thoughts. Few examples are 1) Providing yourself strong signals known as anchor codes 2) Thought stopping techniques 3) Distraction techniques 4) Relaxation exercises 5) Self-statements etc.

Behavioral techniques involve exposure to the dreaded phenomena and prevention of subsequent response. People with mild to moderate behaviour usually need about 10 hours of therapist treatment, combined with exercises to be done at home between sessions. Those with moderate to severe symptoms may need a more intensive course of therapy that lasts longer than 10 hours. These exposure exercises need to take place many times a day, and need to be done for one to two hours without engaging in compulsions to undo them. It starts with construction of hierarchy of anxiety-provoking events. Then exposure is started with the situations that cause the least anxiety. Once you have one exposure task, you can move on to a more difficult task, until you have overcome all of the situations that make you anxious.  It is important to remember it can take several months before a treatment has a noticeable effect. It is extremely vital to remember that no one is perfect, nor can anyone recover perfectly.  Even in well-maintained recoveries, people can occasionally mess up and forget what they are supposed to be doing. People with Obsessive compulsive behaviour find that when they confront their anxiety without carrying the compulsion, the anxiety goes away. Each time, the chances of anxiety reduce and last for a shorter period of time. 

In the second case, treatment is by medication to control one's symptoms by altering the balance of chemicals in the brain. The medications have to be strictly taken under the observation of a psychiatrist.

  1. Setting behavior limits: If you want your kids learn how to lead a disciplined life, then appropriate behavioral limits have to be clearly established for the key behaviors that are expected from them. 

  2. Anger management - You must control your child's anger actively. Otherwise, it will be difficult to treat ADHD, effectively. For anger management, several techniques can be used, but they must be selected after discussion with the mental health worker.

  3. Setting pressure free routine structure - Preparation of sensible routines and scheduled charts is an important step towards ADHD management. However, the same should not become a pressure for the child. Hence the child's suggestions and interests should be kept in mind while preparing a schedule. 

  4. Advocating child - Advocating your child is very much required maintaining a healthy balance for a child suffering from ADHD. Talking about the child’s interests brings the family members closer to the child. In the course of such an interaction, less strictness brings about more meaningful interactions. Typically, a family member or a tutor should be involved in this process. 

  5. Acquiring knowledge about ADHD - Both parents and kids should have the required knowledge for ADHD. Having a proper knowledge will not only aid the parents in early recognition of symptoms and signs, but also in understanding and implementing necessary treatments.

  6. Being persistent - One of the basic principles of behavior therapy is that the interventional technique has to be persistent. The instruction to be given by parents, the application of incentive or punishment based techniques need to happen repeatedly to have a persistent effect on the child. But it is always important to consult a specialist/psychiatrist if you observe that your child’s maladaptive behavior is not changing in spite of your efforts.

For human beings, the body is considered the temple, one that they strive to keep fit and in shape. In this over engagement to restore the physical health, the well being of the mental health is never considered. However truth remains that the health of the mind is equally if not more important and must be given its share of priority. With the growing age and times, depression has emerged as a gnawing problem, one that is plaguing individuals indiscriminately and one that must be eradicated as soon as possible. 

Some of the ways to combat this fatal ailment are: 

  1. Exercises: Everyday a bit of exercise not only keeps the body fit and healthy, but also goes a long way in easing and calming the mind. You need not indulge in strenuous exercises, but must make it a conscientious habit to work out a bit daily.
  2. Talk to people: Though depression makes one inexorably quiet and silent, the first thing that is prescribed as a remedial measure is talk to people. Engaging in conversations with people, telling them what exactly is troubling you are proven ways to combat depression.
  3. Meditate: There is no better way to treat mental ailment than meditating.  When you suffer from depression, your body undergoes severe conflict. Meditating not only calms the mind, but also soothes away the worries.
  4. Develop a support system: Depression is almost always followed by an unfathomable sense of loneliness. Talking to others does help, but at the same time try to develop a support system. Find an individual, be it parents, relatives or friends and confide in him or her. Not sharing what you are going through is never a solution and only exacerbates the fatality of the situation.
  5. Change the daily routine: Monotony is never exciting and it is only time, before the humdrum of life gets to you. I am taking (paroxetine) for anxiety and stress issue from last month. But I am suffering from precum since 7 years. My body. Hands and voice is also shaky. Is this precum related with anxiety. Sperm drops or colourless fluid comes out in seconds when I do normal talk with girls. Even when I sit besides my uni classmates. And I can not perform normal work due to this premature ejaculation prescribe meds plz.

Pre-ejaculatory secretion (precum) is a normal process. It is not a disorder. It is a common misconception to confuse precum with premature ejaculation. The precum is a normal secretion which is secreted for the purpose of lubrication of urethra. Your other symptoms seem to be related to anxiety. Please communicate your concerns to your psychiatrist and get clarification about your condition. In my opinion, with the information that you have shared, there is no evidence of any disorder. So take it easy and consult your psychiatrist or any general physician.

Shivering is a part of physiological manifestation of anxiety. It can just be a normal trait which if is an isolated presentation may not actually require treatment. However, given that you have had multiple such attacks, it may indicate towards an anxiety condition of which there are many. For a proper treatment, the trigger for anxiety, frequency, type of clinical presentation, associated medical conditions etc need to be identified and keeping all these things in consideration, then a suitable form of treatment has to be decided which can be just a simple relaxation technique or a behavioral intervention, CBT technique or psychopharmacological intervention or a combination of some of these. Best call for you would be not to diagnose the issue by yourself which several people try to do and instead visit a qualified psychiatrist. Mostly a proper treatment will relieve you of your symptoms. Wish you all the best.

From what you say, it is difficult to make a diagnosis of bipolar disorder. However, that can be a possibility. Only a detailed analysis of symptoms and mental status examination can provide a clear picture. Before that, we could just be rushing to a conclusion because there are several other conditions which may present with a fluctuating mood which include but are not limited to: emotionally unstable personality, soft bipolar disorders, psychoactive molecule influence, etc. Coming to recovery, it is not clear if he has not shown recovery under medications. If medications were given, there are several issues to be considered for an effective treatment including dosage, augmentation, psychopathology etc. The word recovery may not apply to such a chronic and relapsing condition. However, with proper treatment and follow ups, the number of episodes, severity etc can be considerably decreased. In fact there are thousands of individuals who are living normal lives with proper treatments. The most important step for you right now would be to consult a qualified psychiatrist and get a proper diagnosis. Wish you all the best.

Hi lybrate-user. Neurotic spectrum disorders can present with a variety of symptoms. Some of which have been listed in various books and diagnostic classifications. However, they do not constitute the exhaustive list and thus anxiety and depression can be of several different forms some of which we may not even know. The art of effective treatment is to have a flexible and customized approach. Unfortunately, people practicing one specific technique/system prefer their own ways. But it may not be as simple as that. Therefore, if you do not find help or inadequate support from one way, we should look for another ways. There are many including pharmacotherapies, psychotherapies, meditation based interventions and so on. What helps in a specific case has to be found out. Please visit a near psychiatrist/clinical psychologist and try to find a more suitable/effective technique for yourself under the expert guidance. All the best.
