Dear doctor, I met an accident and fractured occurred in right leg on 29th Dec 2014. As per the X ray report it is “fracture lateral malleolus right(closed) with fracture 2nd and 3rd metatarsal(closed)”. Initially,the doctor thought for surgery for ankle injury but another senior Orthopedic surgeon of the same hospital after examining the X ray plate, put a POP BACK Slab for 15 days and as per his version chance of displacement was there. On 13th Jan 2015, another X ray was done and the doctor surgeon wrote in the prescription ” X ray shows good alignment, no displacement”. On the same day synthetic casting was put in place of POP Back slab and I have been told to meet him after one month. I was allowed to walk with two elbow supported crutches without giving any load to the right leg. Now I am off from my office since more than one month.Although I am obeying all the instructions given by the doctor, i am feeling some pain on the ankle fracture occasionally. Pain is like needle pushing and some sorts of inflammation but tolerable. No swelling is there at present.Rarely I feel tolerable pain on metatarsal fractures. Is this pain normal ? Can I join my office using crutches now ? when i can walk on my feet? How many days it will take further to drive car ? Please note that I am now 49yrs, wt-74kgs normal built, no BP and diabetes. Hello, I am Dr Akshay from Fortis Hospital, New Delhi. I have gone through your case summary. Your injury was on 29th December and it is 2nd Feb today. Since cast was applied on 13th Jan it is also 3 weeks since then. If it is an undisplaced fracture then management is ok. We need to get sequential X Rays to look for any displacement because as the swelling subsides fracture can get displaced because there is looseness in cast. Anyway since conservative management is chosen i would like to have a look at you recent X Rays before giving my verdict. From the last 6 month I have pain in my back bone but some time the was hike, and some time the pain was normal and no any other feels in body, please suggest the best advise.

Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily.

My father having knee problem. We consulted the orthopedic doctor. He suggested should be replace knees. We are OK with that but we have a doubt his age around 62 if we go for knee replacement it will use for future or not. Please let me know. We have seen few after knee replacement still there are suffering and unable to walk also. Kindly let us know for knee replacement.

Hello. You have not mentioned since how long he has been suffering from knee pain. If he has very chronic pain and he has gone through all other alternative ways and does not get relief and his pain is at very serious level he can go for knee replacement surgery ,if his knee bending is too much restricted and unable to walk then knee replacement is advisable, its completely dependent on his body response how he ll recover .As a Physiotherapist I advise you first go for alternative therapies if they don't work out you can definitely go for knee replacement it is a last and convenient option. Stem cell therapy and other therapies are also there but because of age it may not work out but knee replacement may but after surgery exercise and care should be taken. Thank you.

Six Common Causes for Shoulder Pain

The shoulder is one of the most important joints in the body that uses a ball and socket to join the arm to the rest of the body. Any pain can make it difficult to carry out motion in a comfortable manner. The shoulder consists of the long arm bone called the humerus, the collarbone or the clavicle, as well as the shoulder blade called the scapula. A layer of cartilage provides essential padding to the bones in this area and its two main joints. There are a number of issues that can cause shoulder pain. 

Let us get to know the six most common ones. 

  1. Rotator Cuff Injury: This is the most common cause of shoulder pain and it occurs when the four muscles or the tendons are injured. These muscles and tendons usually control the movement of the arm. This injury can involve strains and tears which may also be caused by constant lifting of heavy objects. 
  2. Rotator Cuff Tendonitis: This condition affects the tendons and muscles that help in moving the arm freely. This usually happens due to inflammation in the tendons. It is commonly experienced by patients who are actively involved in sports and other heavy physical pursuits in their line of work. 
  3. Frozen Shoulder: This is a debilitating condition that restricts the free movement of the arm and is also known as adhesive capsulitis. When the shoulder tissues become too thick, it leads to the growth of scar tissue that hinders proper movement and causes pain. 
  4. Osteoarthritis: Osteoarthritis is a condition that comes as part of aging bones that also become hollow and brittle over the passage of time, which leads to joint and bone pains in various parts of the body. It is a degenerative disease that is caused due to prolonged wear and tear as well as other factors including family history and sports injury.
  5. Bursitis: This condition is known to affect the fluid-filled sacs that lie in between the joints. These are known as bursae. When the bursae suffers from swelling, the effect usually shows up as pain in the joints. 
  6. Tendon Sheath Inflammation: The fibrous tissue that connects the bones and the muscles is called a tendon. The presence of the tendon makes it possible to pursue activities like running, jumping, lifting, gripping and more. It is protected by a sheath known as synovium, which also lubricates the same. Any injury or tear to this sheath can cause shoulder pain.

Acute and persistent pain that does not abate and gets even worse should be treated with immediate medical intervention. If you wish to discuss about any specific problem, you can consult an orthopedist.

Morning walk is not an issue but you need to be ruled out for you joints pain, it may be some exertion only but not for sure.

Recurrent shoulder dislocation has to be operated arthroscopically after proper evaluation. How many dislocation have you suffered till now? Repeated dislocation causes more damage every time it gets dislocated. Please visit a shoulder surgeon before damaging you shoulder more. I am 30 years old female. I have neck pain while I taking pillow at night and also while continuously working on laptop. I think it is cervical but I am not sure. The pain extends from neck to back of shoulders and also when I mo while moving neck backwards. I have never taken any medicine regarding the same. Please suggest.

Diet and lack of exercise are the two most important factors that contribute to obesity. While the modern day diet is becoming more and more full of fats, sweets and empty calories, it is becoming devoid of nutritious vitamins and minerals. The fibre content is also decreasing. Hello. Maintain a right standing and sitting posture, keep your back always straight, avoid carrying weight on back. Hold or carry weight divided equally in both the hands or shoulders (to avoid over straining on one particular hand or shoulder). (Excessive sitting or standing is to be avoided as well don’t carry much weight on shoulders). Exercise: Practice Shavasana or Corpse Pose as it looks like a dead body. If practiced correctly and with full efforts, can relax every body part, relaxing not only the body but also the mind: Spread the legs one to two feet apart, the toes are turned outwards, the heels facing each other, a comfortable distance apart. Bring the arms a little away from the body, palms turned upward. Relax the neck and allow it to turn to the side if it is more comfortable. Close the eyes and focus the attention on the body, breathing normally. Begin focusing each body part and relaxing it, then moving on. Keep the mind focused on relaxation, the breath should be normal. Relax the whole body. Management: -Stretching or activities that place additional strain on the back are discouraged. -Sleeping with a pillow between the knees while lying on one side may increase comfort. -Lying on your back with a pillow under your knees would also relieves pain and no pillow under your head. -Some people seem to benefit from the use of ice or heat. Take care, do not use a heating pad on ‘high’ or place ice directly on the skin. Medication: Take homoeopathic medicines - Arnica 200 and Rhus Tox. 200 - take both of them thrice daily for 1 week. Also, apply Schwabe's Topi-Arnica and Topi MP Gel on the affected area in circular motions gently thrice a day. Get back to me thereafter.

. Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Hi. Dr. Mai last six month se family plan kar rahi hun. Plan karne se pehle mere periods ekdam regular they. Jab se plan karna shuru kiya hai tabse periods 5 se 6 days late hote hai. Is baat bahut pain bhi hai. Periods khatan ho gaye hai. Par ab bhi low stomach aur back pain hau. Mai ovacet bhi le rahi hun. Pregnency k liye kya karun.

Period 5-6 din late hone se koi problem nahi hai, ovacet ke karan bhi period late ho sakta hai, agar apne koi test nahi kiya hai to dawa khane se pahle test karna chahiye.

Hi, I have right shoulder pain, hip pain, right thigh pain. From yesterday. I have pcod taking treatment. First time I took siphene (clomifene). Is my pain related to this? OR I am even diagnosed with kidney stone of 3mm. Is my pain due to this? When I get up in the morning I have heel pain. This heel pain is from 2 days. Can you please say whom should I consult?

heel pain could be due to plantar fascitis. the limb pain could be due to harmonal imbalance. take simple analgesics and if they dont help then consult an ortho surgeon
