Needs to be screened by sputum analysis, chest xray and head and neck examination. Pinky sputum may indicate bleeding, from any site, may be bleeding gums, so go for evaluation before getting scared

Hi, I want to ask a question about breast cancer in women, What are the symptoms of breast cancer and its causes?

M.Ch - Surgical Oncology, MS - General Surgery, MBBS
Oncologist, Delhi
Breast cancer usually presents in middle aged to elderly women, with a painless breast lump, or bloody nipple discharge, or a non healing recent onset ulcer on the skin. It may arise as a result of hereditary factors, but women who have not given birth or who have not breast fed, or women having early menarche and late menopause are also more predisposed to having breast cancer in their lifetime.

How to clean a smokers lungs? What are the precautionary measures to avoid lung cancer & throat cancer?

Hi lybrate-user, I appreciate you seeking help. First and foremost, one must quit smoking. Again, it does not necessarily mean that everybody who smokes gets cancer. Smoking does increase the risk, though, of getting cancer. Simultaneously, it exposes one to various lung ailments too. I would advise you to go for a healthy and balanced diet and maintain a regular and active lifestyle with due focus on daily exercises, yoga/ 'pranayam' (/breath control), de-stressing and relaxation. Try including more of deep green leafy vegetables, whole grain cereals, omega-3 fats and fresh seasonal fruits in your daily diet. Preferably, go for a mediterranean diet for maximum benefits. Staying well hydrated is important too. Dietary supplementation of B vitamins especially vitamin B6, in contextually appropriate doses, is known to reduce the risk of lung cancer. The above-mentioned mostly stands good for throat/ oral cancer too. The dietary guidelines apart, including fresh drumstick, neem, cranberry etc is known to contribute to chemo-prevention too. Additionally, do take care of your oral hygiene on a day-to-day basis. Would recommend a regular dental / ENT check up (every 6 months) too, to be on a safer side. Hope this helps. Do take care and all the very best. Sincerely,

How Frequently Should a Breast Examination Be Done?

MBBS, MS - Obstetrics and Gynaecology
Gynaecologist, Noida
How Frequently Should a Breast Examination Be Done?

Breast examination is a way of detecting early changes that help in detecting lumps or other growth in the breast. This is a manual form of examination that may be carried out by the doctor or even by the patient. This kind of examination helps in detecting the onset of breast cancer and helps in successful treatment of the same. It is an essential screening strategy that all women must go through. Let us cast a glance on the various details regarding this examination, and also how frequently it must be conducted.

  1. Age: Women who have reached the age of 18 are said to have matured physically and sexually. They should perform this examination once they have reached the said age in order to detect any anomalies in the way the physical changes occur in the breast tissue.
  2. Procedure: One should begin by looking at one's reflection in the mirror to find any rashes or dimpling in the breasts. The shape, size and colour of the breasts and nipples must also be studied so as to check for any anomalies. Inverted nipples, redness and soreness must also be reported to the doctor. Once you are done checking the visuals, you can raise your arms and look for any changes. Then, you will need to lie down and feel your breasts by using a circular motion. The finger pads must be used in order to check for any lumps. One must start from the centre and move the breasts sideways. Any wet and slippery feeling must be reported to the doctor immediately.
  3. Pressure: During the breast examination, you must use light pressure for the skin and tissue that lie beneath the breasts, while medium to moderate pressure should be used for the middle portion of the breasts. One must use a firm hand to check the tissue that lies at the back of the breasts, near the rib cage.
  4. Frequency: One must get into the habit of conducting a breast examination at least once a fortnight or once a month, without fail. You can also go to the gynaecologist for an examination in case you are not confident of conducting it on your own with proper movements and accuracy. You may also journalise your breast examination findings.
  5. Menstrual Cycles: When you are going through your menstrual cycle or period, it is important to remember that your breasts can become tender just before or after the start of the cycle. So, do not panic if you think you have found a lump or any other growth at this time. It is best to check again after a week and then visit the doctor about checking the same.

Sir my girlfriend's breasts leaks fluid like something of light colour slightly sticking when touch. Neither she is breastfeeding nor pregnant, we enjoyed on 20/2/17 and when I squeeze her breasts something sticky fluid sticks my hand and when I kiss it tastes sweet. Later she informed me that fluid comes out when she squeeze her breasts both but in single little droplet. She have no tumour or burn or pain. Her menstrual period ends on 18/2/17. Pliz acknowledge the symptom. Is she suffering from breasts cancer? I fear sir.

It is usually not breast cancer. There are many ducts in the breast. One of them may start secreting fluid due to various reasons. It should be investigated by a surgeon.

Prostate enlargement requires a full clinical evaluation for the cause of enlargement. It is important to assess ultrasound status and post void residual urine along with serum PSA before starting any medications. If benign hypertrophy is present you can start tablet Urimax D daily once at bedtime.

Respected lybrate-user tokas hi I don't recommend medicines for weight loss at all. Just avoid more oily spicy stuffs, do regular walking if 3-4 kms a day, do some exercises by your own have more salads monitor weight on regular basis sure you will get results. So far as breast discharge is concerned its hormone related but you can primarily go for ultrasonography or mammography of breast definately. Thanks regards.

Dear lybrate-user, fibroadenoma are breast lump usually occurring in 15-25 years age group. In case the lumps aren't large enough, they should not be removed. An ultrasound of both the breast should be done for better diagnosis of the breasts. I would suggest you to consult a surgeon.

Also get a blood exmaination dole to check out her hemoglobin and neitrophilic counts this might be a case of neutrophilic typhlitis. Thanks.

Hi, My father-in-law has been detected with 4th stage Lung Adenocarcinoma, June, this year. He is having a history of Tobacco chewing. PET-CT and CT scan suggested that the cancer has spread to lymph nodes and some part of abdomen. He completed his 4 rounds of chemotherapy with Carboplatin and pemetrexed and he is completely fine and totally symptomless. His blood count is absolutely normal. Can we hope that he has the chances of recovery.

MBBS, MD, CCEPC, FISSP, OBS, NFPM
Oncologist, Delhi
What does his latest PET scan say? Regular PET scans can only tell the ongoing disease status. Its good that he is fine. As long as he can accept chemo without side effects, it may be taken. Stage 4 cancer is difficult to respond 100% with any chemo.

How Laser Prostate Surgery Is Performed?

MBBS, MS - General Surgery, DNB
Urologist, Bangalore
How Laser Prostate Surgery Is Performed?

One of the most common problems faced by ageing men is benign prostatic hypertrophy. Prostate is the gland at the base of the urethra near the bladder and when it enlarges it can lead to symptoms, mostly related to urination.

1. Frequent urge to pass urine
2. Prolonged urination
3. Frequent nocturnal visits to the toilet
4. Intermittent urination
5. Difficulty to start urinating
6. Inability to completely empty the bladder
7. Urinary tract infections

There are medications available to manage this, but offer only temporary relief. Many men therefore prefer to have the surgery undergone to manage these bothersome symptoms. However, like any surgery, the risks and benefits need to be considered along with other conditions like age, overall health status, other comorbid conditions, etc.

Surgical removal of the enlarged prostate gland is a more definitive approach to manage these symptoms. In addition to providing a quick cure, it also is used in the following cases:

1. Patients who do not respond to medications
2. Presence of blood in the urine
3. Associated bladder stones
4. Frequent infections of the urinary tract
5. Associated damage to the kidneys

Procedure of Surgery
During the procedure, a tube is passed through the tip of the penis into the urethra towards the bladder neck. Once it is in the desired position, laser is passed through it to deliver energy that acts on the prostate to either completely or partially destroy it. There are two methods by which laser acts on the enlarged prostate and making way for free flow of the urine.

1. Ablation: Excess prostate tissue is melted away by the laser by using photosensitive vaporization of the prostate. This is also known as Greenlight laser therapy or KTP laser vaporization. Alternately, Holmium can be used as the source of laser energy to ablate the prostate tissue.

2. Enucleation: Excess prostate tissue is cut and teased out through the urethra. Holmium laser is used to resect the prostate into smaller pieces, which are then removed out through a resectoscope. Another technique uses a tissue morcellator which grinds the enlarged prostate into smaller pieces to enable easy retrieval.

More men now opt for laser prostate removal as it has the following advantages:

1. Reduced risk of bleeding: This becomes essentially important in patients who are on blood thinners.
2. No hospitalization: This can be done either as an outpatient or with minimal one day stay at the hospital
3. Immediate symptom relief: As compared to medications, the relief is felt almost immediately after the surgery
4. No catheter: With laser surgery, a catheter may be required for less than 24 hours unlike in open surgical cases.

As noted above, as with any surgery, once enlarged prostate symptoms set in, have a detailed discussion with your doctor to identify if you are a suitable candidate for laser surgery.

MBBS, M.Ch - Plastic Surgery
Cosmetic/Plastic Surgeon, Ahmedabad
lybrate-user, For last 28 years I have been undertaking breast implants. For married women where scar is not a big issue, I offer silicone breast implant surgery. For unmarried, divorced, spinster seeking partner or marriage, I do offer sessions of autologous (your own) fat grafting. Here the scars are much smaller at 2 places: the body area from where we take out fat (Usually tummy, thighs, hips), and over lower part of breasts from where we inject. The results of fat grafting may not be as great as that of implants, but still good.
