Hot & Cold Therapy - How It Can Treat Pain?

MD-Anesthesiology, MBBS , F.I.PM, Fellow in Endoscopic Spine Surgery
Pain Management Specialist, Ahmedabad
Hot & Cold Therapy - How It Can Treat Pain?

Hot and Cold therapy is one of the most popular home remedies for any kind of joint pain or muscle sprain. In some cases applying heat therapy and cold therapy alternatively, can help relieve the pain while for others only one form of therapy is necessary. To understand what to use when, one should first understand the effect of heat and cooling on the body.

Applying a heat pack or immersing the affected limb in hot water improves blood circulation and makes the muscles more flexible. Thus, it can help relax a muscle and fasten the healing of damaged tissues. On the other hand, cold therapy or cryotherapy reduces blood flow to the affected site. This helps relieve inflammation and pain caused by it. It can also make the area feel numb by reducing nerve activity temporarily.

Types of Heat Therapy
Heat therapy can be categorized as dry heat and moist heat. Dry heat involves using saunas or dry heating packs to relieve pain. This is also known as conducted heat therapy. As the name suggests, moist heat involves the use of steamed towels, hot baths and moist heating packs. This is also known as convection heat therapy. Moist heat is usually considered more effective than dry heat.

Types of Cold Therapy
Cold therapy may be applied to a part of the body or to the entire body. This includes ice packs, coolant sprays, ice baths, ice massages and whole body, cold therapy chambers.

How to use heat and cold therapy?
The sooner heat and cold therapy is given to a person, the faster they will recover from their pains. Heat therapy can be given to a person for extended periods of time but when giving cold therapy, a person should not be exposed to a cold pack or ice bath for more than 10-15 minutes. Care should also be taken no to apply ice or a frozen pack directly on the skin as it can damage the skin and nerves.

When not to use Heat Therapy?
If the injury site is swollen or bruised, cold therapy is better than heat therapy. Heat therapy should also not be applied to an open wound. People suffering from conditions such as diabetes, dermatitis, deep vein thrombosis, vascular diseases and multiple sclerosis should also avoid heat therapy as these conditions can increase the risk of burns and other such complications. Pregnant women should also consult a doctor before using a hot tub or sauna.

When not to use Cold Therapy?
Cold therapy is avoidable for treating stiff joints and muscles. People suffering from poor circulation and sensory disorders should also avoid using cold therapy without a doctor’s supervision. If you wish to discuss any specific problem, you can consult a pain management specialist.

I get plenty of sleep. Sometimes I even over sleep. However my eating habits are not regular and not good. I eat junk food and skip meals. Also I forget to drink water. Hye. You actually know what exactly is the reason for your problem. You are losing energy, nutrients and feeling lethargic and sleepy because of poor eating habits and also this is leading to fat gain. There’s no overnight fix when it comes to health, but you can adjust your lifestyle to promote quick, natural weight loss for the benefit of your health and your appearance. Successful, long-term weight management needs to focus on more than the food you eat and the pounds you lose. It needs to focus on your overall health. Reshape your body and your lifestyle by adopting healthy habits and breaking unhealthy ones. Replace all processed foods in your diet with vegetables, fruits, whole grains, low-fat dairy, lean meats, eggs, legumes, nuts, seeds and seafood. Focus on the variety of whole, unprocessed foods you can eat instead of dwelling on the junk food you eliminated from your diet. Eat smaller portions at every meal and avoid going back for seconds. Take a smaller scoop of rice,  half of a piece of chicken instead of a whole piece and one less egg in your morning omelet, for example. Make time for breakfast every morning. Eating breakfast helps prevent overeating and is a habit of people who successfully lose weight. Pack to-go meals instead of eating convenience foods when you’re not home. Store foods like chopped fruits and vegetables or cooked grains, legumes, eggs and meats in your refrigerator so you always have something to grab when you’re in a rush. Buy fresh vegetables and fruits from the grocery store if you forget to bring healthful food from home. When you can’t avoid dining at a restaurant, opt for a salad as a starter, and always ask for the dressing on the side. Clear soups, lean meats, seafood and grain-based dishes like roti, whole grain bread are other good choices. Exercise for 30 to 60 minutes at least three times per week to build muscle and burn more calories. Pilates, strength training, aerobics, swimming, dancing and yoga or a sport like football are ideal choices. If you’re new to exercising, start with brisk walking and jogging. Adopt physically demanding hobbies like bicycling, hiking, jogging, power walking to promote faster weight loss. If you don’t have time for new hobbies, increase your physical activity with things like walking the dog, playing with the kids or jogging instead of driving to a nearby store or taking the stairs instead of the elevator.

Hi thanks for your query and welcome to lybrate. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medications which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we might have to get some investigations like dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail

Dear lybrate-user! Upper arm pain in young age is commonly due to muscle cramps or diabetes which results from 1. Dehydration, 2. Calcium deficiency considering your age its most likely due to dehydration or nutrition related. It will be relieved by simple measures as follows: take adequate liquids, sports drink like gatorade, fruit juices, buttermilk, lemonade etc. 3. You can massage arm muscles and apply cold pack when they pain. 4 avoid prolonged stitching 5.Change postures in between. 6. Take calcium supplements. 7. Take healthy nutritious diet. 8. If you are taking medicines like diuretics, statins, review with your doctor as they can cause cramps too. 9. Consult the nearby homeopath If problem persists get your thyroid profile, kidney functions done and consult with your doctor. Thank you

Dear you probably have a muscle problem. The hamstrings can give pain on getting up and down. Foot pain may be due to planter fasciitis or any other muscle of the foot. Apply ice and give rest to the muscle maybe yoga can help. If not relieved you need detailed evaluation and imaging to come to a diagnosis.
