I am 70 year old male and have been suffering from pharyngitis for over 18 years now. I have not found a cure yet despite my unremitting efforts. The pain on both sides of my throat are severe at times and I am forced to rely on analgesic and anti-histamine medicines. Please help me overcome this suffering.

MS - ENT
Ear-Nose-Throat (ENT) Specialist, Rajkot
I think you may have some problem in nose - either deviated nasal septum or sinus problem. Due to this problem you may have chronic post-nasal discharge wich is causing recurrent pharyngitis. So, it is my advice to take ct scan of paranasal sinuses to find nasal cause.

Respected Sir or mam I have a problem I am snoring too much when I sleep please suggest me. Thanks and regards Rakesh Chakravorty.

MS - ENT
Ear-Nose-Throat (ENT) Specialist, Gandhinagar
Snoring: Snoring is mainly due to Vibration of Pharyngeal parts mainly soft palate parapharyngeal bands base of tongue, Adenoids and polyps in nose etc and also Deviated nasal septum plays the role. It is better to Go for sleep study test and Endoscopy of Nose and Throat in Awake and sleeping pt. Also MRI during sleep helps to come to Diagnosis. Sleep Ap machine can help the pt . Also surgery of Nasal polyp, Deviated Nasal Septum, Tonsils and Adenoids, Soft palate and Base tongue can help to cure or reduce the Snoring….

How Can Nasal Obstruction Cause Clogged Ears?

MBBS, MS - ENT, DNB
Ear-Nose-Throat (ENT) Specialist, Gurgaon
How Can Nasal Obstruction Cause Clogged Ears?

It is very common for patients with nasal obstruction to develop clogged ear as well. The back of the nose opens into a tunnel also known as the Eustachian tube. This tube goes up to the ear. In case the ears get clogged the pressure is released by popping of the ears. This is achieved by swallowing, yawning, and trying to blow the nose. When ear pops the tunnel opens and allows the pressure to release from the ear.

Reasons why nasal obstruction causes clogged ears:

  1. Mucosal Swelling: When the nose is obstructed, the nasal lining might swell thus making it difficult to breath. This might also lead to swelling in the tunnel of the ear causing it to shut down. Sometimes an ear pop does not happen in this condition and this is called as Eustachian tube dysfunction.
  2. Negative Pressure inside the Nose: In nose obstruction when the patient tries to breathe through the nose there is negative pressure. This might build up near the opening of the Eustachian tube or the back of the nose. This pressure gets transmitted to the ear and results in a negative pressure in the ear which leads to the clogging of the ear.
  3. Bernoulli's Principle: This principle states that in case of an inviscid flow, increase in speed of the fluid happens side by side with a pressure decrease.

Causes of Nasal Obstruction:

Obstruction or congestion of the nose leads to a stuffy and inflamed nose. In minor illnesses like fever, cold, sinus infection and flu stuffy nose can happen. This congestion might last for a week. In case the congestion is long term then the causes for it are:

  1. Allergies and hay fever
  2. Chemical and environmental irritants
  3. Septum deviation
  4. Chronic sinusitis that has lasted for too long
  5. Nasal polyps and benign tumors

Symptoms of nasal decongestion are runny or stuffy nose, painful sinus, and swollen nasal tissues along with mucus buildup.

Treatment for Nasal Decongestion:
Once the nasal congestion has been found and the cause behind it has been analyzed, then treatment is must. Treatment usually includes prescription medication or over the counter drugs. Medication of the nasal decongestion involves the following:

  1. Oral Antihistamines: These are helpful in treating allergies. Common ones are loratadine (Claritin) and cetirizine (Zyrtec)
  2. Nasal Sprays: Nasal sprays which contain antihistamine like azelastine (Astelin, Astepro)
  3. Nasal Steroids: Examples of this are mometasone (Asmanex Twisthaler) or fluticasone (Flovent Diskus, Flovent HFA)
  4. Antibiotics
  5. Over the counter or prescription decongestants

I have a problem of nose block since last 2 days and no nasalspray has cured it permanently. What to do?

DORL, MBBS
Ear-Nose-Throat (ENT) Specialist, Faridabad
If a person suffering from nasal obstruction with mild to moderate nasal allergies it means most probably he is suffering from dns (deviated nasal septum. Nasal obstruction is because of the deviated portion of the nasal septum (this is the bony partition of the nose cavity. Allergies are because of the hampered muco-ciliary mechanism (normally mucus is produced in the nose to moisten our nose cavity, oral cavity, and help food bolus to move downward, this mucus is basically moves on the cilia of the nasal mucosa) this mechanism gets hampered and mucus start accumulating in the nose and sinus cavity which is why thick mucus comes from nose in patients of nose obstruction and person develops sinusiits. When a surgeon removes the nasal septum deviation has not been removed properly the symptoms persists, and patient thinks that the nose bone has again regrown. It is not because of regrowth but because of half surgery done or due to the non removal of the deviation (reason can be many) nowadays nasal septal deviation can be removed endoscopically.

Ear wax is formed in outer portion of the ear and if you use buds which are not ear buds though, this will introduce the wax further in side the ear and this will lead to blockage, wax is produced on daily basis and if you let water go in side your ear this will help you cleaning your ears by litself and secondly ears get cleaned by chewing your food properly

I read somewhere that wearing headphones for an hour can increase the bacteria in your by 700 times, I wear headphones daily for a minimum of 5-6 hours, I am also suffering from tinnitus which may have been caused by headphone use, please suggest how to handle hygiene issues caused by overuse of headphones.

MBBS, MS - ENT
Ear-Nose-Throat (ENT) Specialist, Ahmedabad
Ear canal is dark and narrow recess so air and light doesn't reach inside properly so over it if you cover it for prolonged period regularly than the issue worsens. Try to use headphones which doesn't go inside the ear but remains over it so some air escapes inside. Reduce duration of use and remove frequently while using. Tinnitus may be a bad sign caused by early damage to hearing nerve. Get a report called audiogram done and stop listening to loud sounds.
