In my professional opinion, your poor vision is not related to your taking thyronorm for 2 months or to your TSH high TSH. But I would definitely investigate further in your case.

In USG NECK, right lobe of thyroid 24x11x09 & show tiny hypeochoic lesions. Is it thyroid nodule or something else?

Yes, it is a thyroid nodule. I would recommend a Thyroid Scintigraphy, and an FNAC to rule out Malignancy.

Hi doctors, my HbA1c test result done in November was 10 which was when I was diagnosed with diabetes. Now after changing my lifestyle with daily exercise and gym I retook the test yesterday n it has come down to 8.3. Even though it is still high, I am hoping to get it the normal range without any meds. Ypu should avoid sugar, sweets,jaggery, honey,sweet fruits like grapse, mangoes,cheekoo, banana etc. Take small frequent meals instead of large ones. Exercise regularly. Monitor blood sugar regularly If still not controlled then add medicines under supervision of a Physician.

Both are high and not in normal range. TSH 5.99 is suggestive of hypothyroid and WBC 12900 is suggestive of acute infection.

You are in early stage or developing hypothyroidism as T3 and T4 is not reaching tissues even if it is high in blood. May be due to antibodies and end organ failure.

Can Endometriosis Cause Infertility & Miscarriages?

There can be a number of reasons that lead to infertility in women. Endometriosis is one of them. It is a benign condition where the endometrium, which includes the tissues that form the lining inside the uterus, grows outside the uterus. Apart from infertility, it may also cause pelvic pain in women.

There are no such symptoms of endometriosis apart from pelvic pain and it cannot be detected without surgery. Thus, it is very difficult to diagnose this disease and state how common it is. However, research says that the prevalence of this disease in women in their reproductive age is between 3 to 10 per cent.

It is found that endometriosis sometimes leads to infertility. Here is how:

Abdominal Adhesions and Infertility
Normally, the endometriosis implants develop in the abdomen. Then the body surrounds them with connective tissues. The reason for doing so is isolating the implants, so that they cannot harm your body.

Now, sometimes these adhesions can create a blockage in the opening of the fallopian tube or may also pinch off the fallopian tube. This will result in obstruction in the fertilisation of egg and the sperm and thus fertilization will not take place; so there are no chances of conceiving. These obstructions when diagnosed can be treated with surgery.

Secretions from implants
Endometrium plays a vital role in conception where the tissues secrete a wide variety of hormones and nutrients. Now, the endometrial implant also functions in the same way and secretes those hormones. The difference is that it does not deposit them in the lumen of the womb, and it is released in the abdominal cavity. In these substances, there may be some hormones that are responsible for infertility.

Prostaglandins
Prostaglandins are other hormones that are secreted by the endometrium normally. It helps in a lot of body processes that include various stages of menstrual cycle and pregnancy.

This hormone is required for a lot of functions such as ovulation, sperm mobility, and regression of corpus luteum, immune interaction, menstrual cramps and contraction of the uterus at birth.

Just like the endometrium, the implants also secrete these hormones. And the problem that is caused by the implant secreted prostaglandin hormone is that they are released in the abdomen and not in the womb. Moreover, they are not secreted at the right time, thus sending a wrong message to the brain.

A woman with a few days of pregnancy may face a miscarriage because the ovary will get a signal to start the menstrual cycle, and the womb lining where the egg is implanted will be expelled.

From the kind of blood levels you have mentioned it is likely that you would need Insulin. Prepare yourself mentally for this possibility. Sapanaji, Thanks for the query. For hypothyroidism only treatment is to take adequate dose of levothyroxine tablets and maintain TSH with in normal limit. Your present TSH level is good. As regards CBC, there is no real problem, you seem to have a bit less RBCs. For that you should take iron supplement tablets. Thanks.

Arhiji, Thanks for the query. To bring down TSH levels to normal it takes at least 6 weeks after starting the treatment with adequate dose of Levothyroxine. When on treatment TSH should be maintained around 3 to 4 mU/L. At that level all the symptoms of hypothyroidism are in check. However, if you want to conceive, then it is essential to maintain TSH level closer to 2 mU/L. Further, during pregnancy TSH should be maintained as follows: 1st Trimester: 0.1 to 2.5 mU/l, 2nd Trimester: 0.2 to 3.0 mU/L & 3rd Trimester: 0.3 to 3.0 mU/l. These levels should be critically adhered to. As lack of thyroxine affects fetal brain development in the first 28 days, later, fetal growth also depends on adequate Thyroxine plus the desired outcome of pregnancy. Thanks.

Mr. lybrate-user, Thanks for the query The blood glucose levels mentioned (particularly fasting) are high. In a well controlled diabetic person fasting blood glucose should be < 100 mg, PP 150 to 160 mg & Hb A1c % <6.8. At this level one is able to either postpone or prevent the long term complications of diabetes developing. Therefore, it is clear that current treatment is not really helping you to control the blood glucose levels effectively. This can lead to developing long term complication Thanks.

Mr. lybrate-user, Thanks for the query. There is no way to tell how many calories one needs to burn if blood glucose is 200 mg/ 100 ml. Caloric requirement is based on actual body weight, extent of daily exercise and may be blood glucose levels. Blood glucose level can be brought down by proper drug treatment, diet and exercise. Mr. lybrate-user, Thanks for the query. Diabetes (whether T2 or T1) needs appropriate treatment with suitable drugs, restricted diet and regular exercise to maintain near normal weight. There is NO CURE for diabetes. It is essential to control blood glucose very effectively. Fasting glucose should be ideally < 100 mg, PP 160 to 170 mg & HbA1c% <7. This helps the patient to either postpone or delay the long term complication of diabetes. If not then there can be very severe consequences involving multiple vital organs and systems. Since no details are mentioned about blood glucose levels, it is difficult comment about the disease and its treatment. Thanks.

lybrate-user, Thanks for the query. I have seen the details, following are my observations: 1)With a BMI of 39.6 kgs/M2 (normal <23 kgs/ M2), you are morbidly obese. This is the main reason for getting diabetes and not being able to control effectively. 2) There is no mention of actual fasting, PP glucose levels and HbA1c% it is not possible to talk about proper control of the condition. 3) There is no mention of dosage of drugs being taken, further no mention of extent of daily exercise, dietary pattern 4) No mention of lipid profile too. My suggestion is please come back with all the necessary details mentioned above, preferably in a private (paid consultation) conversation then I will be able to guide and prescibe if there is a need Please understand almost all the medicines have excellent action, however, there may be a proper drug selection required some times. lybrate-user, Thanks for the query. TSH 220 mU/L is too high, suggesting there is a severe deficiency of thyroxine It needs immediate treatment with levothyroxine. Dose depends upon your height, body weight, TSH & T4 levels plus age.
