Anterior Cruciate Ligament Injury - Know Everything About It!

M.CH. (Ortho), MS - Orthopaedics, MBBS

Anterior Cruciate Ligament Injury - Know Everything About It!

In general, anterior cruciate ligament injuries, or ACL injuries, are understood to be tears in any of the several knee ligaments joining the upper leg bone and the lower leg bone. This can vary from minor injuries, such as small ligament tears, to more serious cases, like complete tears or when the ligament and one of the bones gets displaced from the other. These injuries usually occur during sports activities, like soccer, basketball, football, gymnastics, tennis, volleyball, etc. An untreated ACL injury can lead to a condition called an ACL deficiency. This can cause a lot of problems with regard to knee movements as the bones may rub against each other, causing a lot of pain. In some cases, the cartilage covering the ends of the bones may get damaged, eventually tearing and trapping the cushion pads supporting the knee joints.

Symptoms

Signs and symptoms of anterior cruciate ligament may include:

  1. You are unable to move properly.
  2. You experience immense pain while walking up and down stairs.
  3. You are unable to engage much in physical activities.
  4. Your knee begins to swell gradually.
  5. You can sense a loud popping sensation when moving your knee.
  6. You experience feelings of instability.

Causes
Anterior cruciate ligament injuries generally occur during sports. Here are some common possible instances that might happen during a game:

  1. Sudden arrest in motion
  2. An unsteady landing from a jump
  3. Direct blows to the knee from tackles
  4. Sudden shifts in direction

Treatment
Treatment of ACL injuries normally involves the following:

  1. Immediate medical attention along with intense first aid care
  2. Sincere adherence to the R.I.C.E model of self care -
    • Rest: Proper rest for a couple of weeks or months depending upon the severity of the injury
    • Ice: Using ice packs to help cool the nerves
    • Compression: Ice treatment further helps in compression
    • Elevation: When lying down in bed, it is strongly recommended that you keep your affected leg at a slightly higher position, through the use of pillows or cushions
  3. Success rehabilitative therapy for several weeks
  4. Anterior cruciate ligament surgery or reconstruction

Hi there! Your mother probably suffers from and is being treated for Rheumatoid Arthritis. As a rule, we do not chase CRP levels for a patient. We chase the symptoms and aigns i.e. The problems actually faced by the patient. CRP levels might or might not come down, with or without medicines. But there is nothing wrong with it. I suggest you stop getting repeated CRP levels assessed at your level. You'll be much happier. Let CRP levels be assessed, as and when required, by your Doctor. If clinically o/ symptomatically, your mother is better. That's the best thing. If she still has problems of pain, stiffness, etc. Her medication might have to be increased. Consult as required. All the best!

There are different glands that causes production of saliva. Improper Sleeping Position – When you sleep on your side or your stomach, your mouth often opens as you relax, and saliva can dribble out. But when you sleep on your back, you naturally swallow during sleep, which prevents drooling. Allergic rhinitis and certain food allergies cause over-production of saliva resulting in drooling. Although, I have treated many patients having same probs, so fix an appointment.

Initially try to change your life style and wait for some time. Please check what might be the cause for this. Maintain good sleep hygiene. Adapt a holistic approach towards health. Therefore you need to eat healthy food - have daily fruits and vegetables, exercise every day, avoid stimulants (includes tea, coffee, tobacco, alcohol etc ). Sleep at the same time every day so that your sleep rhythm is not disturbed. Avoid all kinds of gadgets as they emit radiations and can affect the sleep pattern. Yoga and pranayam can be very beneficial as they can relax both the mind and the body. Read more about healthy lifestyle and slowly implement the good habits. You can add Homeopathy if you still suffer from insomnia. You can contact me for a private consultation if necessary.

Chronic Sleep Disoreder - 3 Ways It Impacts Sex Life

Chronic Sleep Disoreder - 3 Ways It Impacts Sex Life

Apart from affecting your memory, health and looks, sleep deprivation can lead to decreased sex function as well. People who have trouble sleeping have a difficult time coping with their relationships as with almost all aspects of their lives. Similarly, in the modern world of continuous work pressure and fast paced lifestyle, stress generally takes away all the energy required to enjoy a fulfilling sex life.

The following are some of the ways chronic sleep disorders and stress may affect your sex life:

  1. Mood swings: Sleep disorders often lead to perpetual mood swings. It can make a person grumpy and quarrelsome. If you're sleep deprived, you may get very agitated even by minor stress. This can hurt interaction between you and your partner, leading to a disruptive relationship in the bedroom.
  2. Anxiety: Increased levels of anxiety and stress attributed to lack of sleep can lead to loss of sexual function and even sexual disorders at times. Lack of sleep generally decreases your experience of positive emotions, which may lead to pessimism and subsequently lack of sexual interest.
  3. Decreased social activity: Lack of sleep may decrease your likelihood of engaging in social interactions. People with sleep disorders usually avoid social engagements, as they believe it may disrupt their sleep patterns. They rearrange their sleeping patterns by avoiding social interactions, which leaves them with lesser opportunities to meet with people of the opposite sex, thereby affecting their sex lives.

So, what you should do?

  1. Focus on getting rid of other stressful issues from your life: Common tensions of your daily life and everyday issues like the normal household chores, and your duties and responsibilities towards your family often leave you too exhausted to spend time with your partner. Moreover, getting preoccupied with social commitments also minimises the time available for just the two of you.
  2. Go to sleep and wake up at roughly the same time everyday to tune your sleep cycle. Avoid waking up late into night as far as possible.
  3. Avoid afternoon sleep or take only a nap.
  4. Don't take heavy foods, big and late meals.
  5. Eat at least 2-3 hrs before sleep.
  6. Avoid coffee and tea in the night.
  7. Don't get glued to the tv, computer, or mobile till sleep time. Shut them down 30 mins. before retiring to bed.
  8. Read something calming, meditate, or listen to quiet music just before sleeping.
  9. Regular workouts helps you get good sleep.
  10. Avoid serious discussions, work or complex decisions 2-3 hrs before sleep. If any of them bother you much, jot them down on a paper. That will lighten your mind help you get better sleep.

If sexual problems and concerns make you sleepless, seek help from a sex counselor. If your sleep problems are due to work in shifts or night-only shifts, take the help of a specialist in sleep problems. This way you can overcome sexual problems arising out of sleep disturbances, and have an improved sex life.

Fractures - Can You Avoid Them?

An average person experiences two fractures during his or her lifetime and same holds true for joint related injuries. The severity of this condition depends on a number of factors, ranging from the forces responsible for injury and location to the damage done to the nearby tissues and bones.

How age plays a role in your chances of getting a fracture?

Your risk and severity of developing a fracture, depends, to a certain extent on your age.

A very common occurrence during childhood is crippling joint related injuries, the fractures that you tend to have during this time are generally less complex than the broken bone instances that you stand to experience when you enter adulthood.

With time, your bones become fragile and you become prone to broken bones sustained from falls, which you wouldn't when you were young. Furthermore, as you step into your 50th year, you can get struck by the bone condition osteoporosis, a leading cause of bone fractures during this time. For women, menopause makes them more susceptible to osteoporosis (as infrequent periods and hormonal changes at this time lead to loss of bone mass) and subsequently broken bones.

Preventing crippling joint injuries need many steps in younger generation known as prehab especially for sporting population and adult population involved in day to day activities requiring your body getting subjected to physical stress.

Simple steps to get your joints back to normal in case you do get into injuries.

  • Having a calcium and vitamin d rich diet to strengthen bones
  • Exercising to strengthen bone and muscle health as well as your balance
  • Taking relevant medicines to make your bones strong
  • Going for timely bone mineral density test to determine the health of your bone
  • Exposing yourself to the sun for about 20 minutes everyday
  • Having a requisite calcium intake of 1000 mg and 1200 mg for pre- and postmenopausal women respectively
  • Preventing a fracture by modification in your household furniture, extra clothings, sometimes addition of simple orthotic devices, improving your muscle reaction time etc go in long way to help prevent falls.

I got my ankle twisted on 5th october. The x-ray showed no bone fracture and the doctor said its a ligament injury. I was having a temporary plaster for 7 days. After that I used crap bandage to walk. But then also I was having pain so consulted another doctor and he put an ankle cast for 15 days then also I didn't got any relief. In having right leg ankle pain i.e after waking up from sleep or after standing from continuous sitting posture.

MBBS
General Physician, Fatehabad
In having right leg ankle pain i.e after waking up from sleep or after standing from continuous sitting posture.
Hi, ankle pain maybe due to various issues. Get a X-ray ankle done to find out for any bone or ligament problem. Sometimes ankle pain may occur due to sprains that took place unknowingly. Ligaments that are attached to the ankle either tear partially, or fully rupture. The overall pain experienced depends on the severity of the injury, yet home remedies can reduce the amount of pain and discomfort you feel. Here are 5 home remedies for you to reduce pain and discomfort on your ankle: 1. Icing: your ankle may respond better to an alternating hot-and-cold therapy. Use two gel packs. One should apply heat for 20 minutes. Follow it with a cold pack for 20 minutes. Alternatively, you can prepare ice packs by filling a gallon-sized Ziploc with fresh ice. Apply the ice pack on your swelling ankle to reduce the amount of swelling will reduce your discomfort. Ensure that the ice rests directly on the swollen area. If the cold is too intense, place a small rag between the skin and the ice pack. 2. Herbal Remedies: Home remedies such as coconut and garlic oil help reduce pain and swelling when they soak directly into the skin. Mix 3 table spoon of coconut and garlic oil into a container and heat in the microwave for ten seconds, slightly warming the mixture. Generously massage the oil directly onto your ankle for ten minutes, allowing the oils to soak into your skin. Cabbage contains minerals and vitamins that are useful when ingested, as well as when they’re applied externally. Gather the outer layers, or leaves, of the cabbage plant. Blanch the leaves by boiling them in water and then immediately placing the leaves into a container of ice-cold water. Once the leaves have been blanched, wrap your entire foot and ankle, and allow them to rest for 30 minutes before removing. 3. Dieting: Changing your diet to relieve ankle pain can be effective over a longer term. If you are heavy, you may experience poor circulation and heightened stress in the ankles and other weight-bearing joints. If you are overweight and suffer from frequent strains or bone degeneration, losing weight can permanently end your ankle pain. Adding a joint supplement to your diet, such as glucosamine or chondroitin, may also help some people by protecting the ankle joint and surrounding tissue. 4. Exercises: Exercise is an ongoing home treatment that will serve as pain relief and prevention. To prevent future ankle injuries, and to strengthen an ankle that is already injured, range-of-motion exercises and stretches provide the most benefit. Only stretch or exercise the ankle once the pain has subsided. Range-of-motion exercises will increase the flexibility of tendons and ligaments that have tightened during an injury. Trace the alphabet with your toes, moving the ankle as much as possible throughout. Repeat at least ten times per day to strengthen your ankle and improve your flexibility. Hope this helps. Good luck.

Dear Lybrateuser, - Sleepiness in the day could be due to improper diet leading to weakness due to anemia, blood sugar changes or hormonal as in hypothyroidism, sleep apnea or sedentary habit & lack of exercise - have a well balanced diet with more of fruits & vegetables including green leafy & seasonal ones, whole grains instead of refined ones, combine protein food with carbohydrates in each meal, also have a snack in between meals, take a walk for 10-15 min after meals for proper digestion & to keep alert - have plenty of oral fluids including 7-8 glasses of water for proper hydration as even mild dehydration will cause tiredness & inability to concentrate - do regular exercise like walking & yoga for 10-15 min in, he morning to keep fit & alert in the day - check blood hemoglobin, sugar & thyroid hormones (panel) to rule out any other problem.

Hi lybrate-user, Using natural treatments for joint pain and stiffness can make a big difference in the amount of medication you need to take. We’ve outlined some great herbal remedies to relieve stiffness and pain. 1. Cayenne - Known for its spicy-hot taste, cayenne makes an excellent topical ointment that relieves joint pain. Rubbing cayenne on the affected area causes a mild irritation, which in turn “distracts” the nerves from the more severe joint pain. Repeated topical applications of cayenne pepper can reduce arthritis pain significantly. To make a topical paste, mix 2 tablespoons of cayenne pepper with 1/2 cup of cocoa butter, lanolin, or coconut oil. Apply it directly to the sore joint. You can also mix 1/2 teaspoon of cayenne pepper with 1 cup of apple cider vinegar and soak sore hands or feet for 20 minutes to reduce pain and inflammation. 2. Fruit Pectin & Grape Juice - The combination of pectin, which is found in the cells of many plants, and grape juice can be of great use to people suffering from inflammation and joint pain. Grape juice is loaded with antioxidants, among them, anthocyanins, which are noted for their effect on reducing inflammation. Pectin is also believed to relieve fluid buildup in the joints of arthritis sufferers. It’s best to purchase pectin from a health or natural foods store for the best quality. You want to make sure to select pectin that is free of MSG and other additives. Mix 1/2 cup of grape juice with 2 tablespoons pectin. You can add water, if needed. Drink it twice daily for 6 weeks, and then reduce the frequency as symptoms disappear. 3. Flaxseed: Omega-3s are important for a strong immune system, but did you know they also help fight inflammation? Flaxseed is one of the best vegan sources of Omega-3 fatty acids, which makes it even more important for arthritis sufferers. Animal fats can often lead to more inflammation, so making the choice to incorporate flaxseed into your diet will help you get the important Omega-3s without adding the animal fat. Try to include two tablespoons of flaxseeds or flaxseed oil in your daily diet. You can add ground flax to cereals, yogurt, and even sauces and stews. 4. Exercise: Light weight training and cardiovascular exercise are very important to relieve joint stiffness and pain. While the first thing you may want to do is curl up and lay down when joint pain strikes, it’s a great idea to get up and get moving. I find that a hot shower and some gentle exericise like yoga, Pilates, or light weight training really helps get your joints in motion and reduces the stiffness and pain. Light weight training can also strength the muscles surrounding your affected joint, thus providing it with more support. Another great form of exercise to get relieve joint pain and stiffness is aquatic exercise. This non-impact workout allows you to slowly moving your joins through their range of motion. Hope this helps. Good Luck.

Thyroid .issues as well as anxiety issues and sleep issues are lifestyle issues which mostly require a lifestyle solution and not chemical cocktail of medicines. You have to address the root cause and not the symptoms and the focus should be on whole patient and not the disease. Hypothyroidism is a misdiagnosed and wrongly treated treated all over the world. Treating only on the TSH level alone is wrong .Dr is treating the labs and not the symptoms .In several cases TSH levels may be normal but patient still has symptoms. Secondly what you have been given is T4 or synthetic thyroxine which is not the active hormone T4 Thyroxine -Inactive hormone T3-Triodothyronine- Active hormone T4 needs to convert to T3 in the liver, kidney and in the thyroid for the conversion to occur you need nutrients which I am sure your Dr. Might not have explained and also you would not have asked. So first thing send me the thyroid profile tests you have done .I am sure they would have left out certain tests that is my usual experience Read up about he disease first and get educated to question your Dr. Do not take your drs words for granted. All your issues are interrelated which can be easily fixed provided you change your lifestyle and eating habits If you want specific advice on what is the correct tests and treatment to be followed and how to make your thyroid working for you and not against you Send a feedback with your name and working phone number and I will call you from abroad and then you can take a consult through Lybrate. Remember you have to treat the root cause and not the symptoms.

Sir can you tell me how many sleeping tablet can I take?
How many mg can I take for good sleep.
It's a wrong information that anyone can take sleeping pills. They are not to be taken on your own. It has to be decided by the doctor. So the best alternative is to exercise everday day which will relax the mind and body and induce natural sleep. A healthy diet is also very important. Read more about healthy lifestyle (from health magazines or from the internet) and adapt good habits. Avoid all bad habits. Learn yoga and pranayam which can improve not only sleep quality but enhance strength and stamina.
