Silent Thyroiditis - Signs You Might Be Suffering From It!

The thyroid is a small gland that controls metabolism and growth. Silent thyroiditis is a common autoimmune disorder that affects the thyroid gland. It causes an inflammation of the thyroid gland and is characterized by mild symptoms of hyperthyroidism and hypothyroidism. It is known as Silent thyroiditis because this condition is not painful in any way. Silent thyroiditis is also known as lymphocytic thyroiditis or subacute lymphocytic thyroiditis.The cause of this condition is unknown and hence it is not possible to prevent it. This is one of the least common disorders to affect the thyroid gland. However, middle aged women are at the highest risk of suffering from this condition. Other risk factors include a family history of thyroid disorders, certain autoimmune diseases, removal of the adrenal gland or treatment for cancer.

Some of the symptoms of silent thyroiditis are:

  1. Fatigue
  2. Increased sensitivity to heat and cold
  3. Erratic bowel movements
  4. Irregular menstrual periods
  5. Increased appetite
  6. Palpitations
  7. Irritability and nervousness
  8. Unexplained weight loss
  9. Excessive sweating

If you notice such symptoms, you must consult a doctor. To determine a silent thyroiditis diagnosis, a thorough physical examination and a few tests are required. The physical examination focuses on the size of the thyroid gland, heart rate and presence of tumors if any. A blood test will also be required to check thyroid hormone levels such as T3, T4 and TSH levels. Your doctor may also ask for an ultrasound of the thyroid gland and an imaging test to determine iodine radioactive uptake.

Silent thyroiditis has a good prognosis and patients usually recover within one year. The acute phase typically lasts only three months. Most patients do not require any form of treatment for this condition but regular checkups are advised to keep track of the disease. In rare cases, medical treatment may be required. For example, if the patient complains of increased heart palpitations or excessive sweating, beta blockers may be prescribed to lower the heart rate. The patient is advised to get plenty of rest and avoid any stressful situations.

In some cases, silent thyroiditis may lead to permanent hypothyroidism. In such cases, medication may be prescribed to normalize the thyroid hormone levels. In some cases, thyroid hormone replacement therapy may also be advised. Surgery and radioactive iodine treatment a not required to treat silent thyroiditis. By the time a woman is 20 years of age, monthly periods become a part of her routine normal life. Most women are well-equipped to handle it without their normal life (personal or professional) getting affected. They know it is “that time of the month” and are prepared mentally and physically. However, some women find it difficult to do it, as they could be suffering from menstrual problems including irregular periods and uterine bleeding. Most often, the bleeding is abnormal and dysfunctional, when most women are not able to identify a set pattern to it.

The list below identifies the most common forms of abnormal or dysfunctional uterine bleeding:

  1. A normal menstrual cycle occurs with a gap of anywhere from 21 to 35 days. Anything shorter than 21 days or longer than 35 days is defined as irregular period
  2. Complete lack of period for up to 3 to 5 months is termed as amenorrhea
  3. Bleeding that is heavier or lasts longer than usual, and occurs before the due date for that month’s period
  4. Scanty bleeding between periods
  5. Spotting or bleeding after sexual intercourse
  6. Spotting or bleeding even after menopause

Diagnosis: Menstrual problems are very common; however, most women tend to shy away from talking about it and do not come out in the open. When talking to a doctor, a detailed family history, menstrual history, and physical examination are done to come to a diagnosis. Additional diagnosis can also be done by ultrasound, hysteroscopy, endometrial biopsy.

Treatment: Identifying the problem helps in initiating the right treatment. A combination of the below is usually used in most women.

  1. Hormonal therapy is based on the underlying condition, and the type and dosage is determined based on it. It could be in the form of tablets, injections, vaginal applications, or released through an intrauterine device.
  2. Non-steroidal anti-inflammatory drugs (NSAIDs) like ibuprofen are useful in reducing bleeding and controlling cramps during excessive menstrual bleeding.
  3. Antibiotics may be useful if there is infection of the pelvic organs
  4. Polyps, cysts, and cancers can be removed surgically and cancer should be ruled out
  5. Endometrial ablation where the endometrial tissue is treated with heat can be used to control bleeding permanently.
  6. Hysterectomy is the last resort where other forms of treatment fail.

The human circulatory system is a highly complex and well-orchestrated system designed with two sets of channels. The arteries are a chain of vessels which carry oxygenated blood to all parts of the body from the heart. The venous system, on the other hand, carries impure or deoxygenated blood from each of the end organs to the heart. These start from minute branches in the distant parts of the body and gradually grow in size until they reach the heart.

These two channels do not interact unless either created artificially or in disease conditions. Naturally, these could be due to developmental anomalies or as a result of an aneurysm and would require a surgical correction. The AV fistula is, however, commonly created naturally in people who have to undergo hemodialysis.

Why an AV fistula: Generally, an artery in the forearm is sewn into a vein in the forearm. This AV fistula may take about 4 months to heal or mature, and a needle is then inserted into the vein during dialysis. When an AV fistula is created, there is a decrease in the peripheral resistance as the blood does not have to exert as much pressure as before, and there is an easy movement of blood into the veins.

How to care for it:

  1. There could be initial limitations on the amount of weight that can be lifted
  2. Keep the dressing in place for 1 to 2 days
  3. Do not sleep on the fistula arm
  4. Do not wear jewellery on the arm
  5. Always wash hands before touching the access
  6. Avoid lifting heavy objects using the hand
  7. Avoiding damage to the arm like cuts or bruises
  8. Regular checking for the pulse or thrill in the arm
  9. Monitoring for signs of infection (redness, blood clots, swelling, pus formation, etc.)


  1. Uses the body’s own blood vessels, no artificial material (as in catheter for instance)
  2. Reduced chances of clotting
  3. It provides good blood flow for dialysis
  4. Is more effective than other types of access (graft or catheter)
  5. Lesser complications compared to other access types
  6. Is done on an outpatient basis and are less expensive
  7. Usually last longer than other access methods


  1. Longer maturity time (sometimes up to 4 months), so are not available for ready use
  2. Stenosis or narrowing of the vessels is common, sometimes leading to blood clots.
  3. If they fail to mature, a fresh fistula may need to be created
  4. Usually created on the forearm, so are quite visible
