In 2nd stage Pile mass comes out while passing stool and gets reduced by itself while in 3rd stage Pile mass needs to be reduced manually. Medicines have very little role in both of theses stages except than regularizing bowel movement. Ksharsutra ligation of Pile mass is necessary in 2nd and 3rd stages.
