My brother's wife is cancer cured - left breast, more than 8 years. She is healthy now, working as well. I would like to know whether her daughter, 26, is taking medication for thyroid, may have this disease on hereditary basis, in future.

DNB, MBBS
Oncologist, Faridabad
See, to ascertain whether it was genetically related she has to check her BRCA genes, usually BRCA 1and 2both are checked and if positive then chances of inherited breast cancers are high. Anyhow, it may be slightly psychologically troublesome as not all cases will develop cancer and there may be other risk factors aswell. She should still go for regular breast self examination on monthly basis, five days after periods are over and can go for mammography after may be 10years as she is only 26.

How many reason of blood cancer? Which type medicins are available for blood cancer in india? How many percentage for live patient? And please tell me Ayurvedic medicine for blood cancer.

DNB, MBBS
Oncologist, Faridabad
Dear mam, blood cancer is usually caused by some mutations or change in genes which control the growth of blood cells. Blood is composed of multiple cells, rbc, wbc, platelets, plasma cells etc. Treatment depends on type of cells effected. Leukaemias have many long term survivors and is nearly curable, but one has to be tolerant enough to complete the treatment and be on regular follow up. I am not aware of ayuvedic medication.

Papillary carcinoma thyroid After removal of thyroid gland, it's there any chance that the disease may relapse?

DNB (Radio Diagnosis), MBBS
Radiologist, Hyderabad
Papillary carcinoma is one of the well curable cancers known. It is a slow growing tumor and complete resection will give a good chance of cure. However Due to long life expectancy there is a 35%chance of relapse over 40years after the surgery.

. I am 26 Years old. I have one friend of same age who gets pain in her breasts now and den. Thing to notice here is he is not getting that pain only during periods but most of the time. She had even felt lumps sort of thing there. please suggest why this happens? And is it very serious? Help us to know more abt this.

Fellowship in Minimal Access Surgery, MS - General Surgery, MBBS, FICS
General Surgeon, Delhi
It looks like your friend is having fibroadenosis of breast. It is a benign condition and not a uncommon. Usually settles down with time. If no definite lumps felt it just needs reassurance only. If your friend has a well defined lump in the breast, a surgical consultation should be sought.

Am I more at risk if my relatives have cancer and what do cancer stages and grades mean?

DNB, MBBS
Oncologist, Faridabad
Dear sir nearly 5-7 % cancers may be inherited, not all, but there can be common risk factors among close relatives like some industrial waste, soil, water pollution etc. Stages means the local or distant spread of disease. It tells about how big the tumour is and where else the disease has gone. Grade means how much the cells have changed from normal cells, well differentiated with better prognosis and poorly differentiatiated with worse prognosis.

Allergy or red spots most likely indicates some kind of allergy and not primary symptom of cancer, red spots sometimes develop if platelets are low in some types of blood cancers but primarily you should consult dermatologist to rule out allergy or worm infestation.

MD - Oncology

Do a biospy from the lump and then colonoscopy with a surgeon, then let me know, even cancer of anal canals are curable, but this can be a papiloma too.

All about Neuroblastoma

European Society for Medical Oncology certification, DM - Oncology, MD - General Medicine, MBBS

All about Neuroblastoma

Most commonly occurring in children aged 5 or younger, Neuroblastoma is a type of cancer developing from immature nerve cells most often found in and around the adrenal glands. However, it is not limited and can develop in several other areas of the body, like the chest, neck, spine and different areas of the abdomen where nerve cells can be found in clusters.

Symptoms:
Depending upon the area of the body affected, signs and symptoms may include:

1. Neuroblastoma in the abdomen:

  • Abdominal pain
  • Diarrhea, constipation or other changes in bowel movements
  • A lump of mass under the skin which is anything but tender

2. Neuroblastoma in the chest:

  • Chest pain
  • Wheezing
  • Visible changes in the eyes, such as drooping eyelids or unequal pupil size

3. There are other signs and symptoms caused by neuroblastoma. These may include:

  • Lumps of tissue found under the skin
  • Bruise-like dark circles visible around the eyes
  • Fever
  • Proptosis (a condition in which eyeballs seem to protrude from the sockets)
  • Back pain
  • Bone pain
  • Unexplained weight loss

Causes: Neuroblastoma typically originates in neuroblasts. These are immature nerve cells formed by the fetus as part of the development process. Eventually, neuroblasts convert into fibres and nerve cells which then make up the constituents of the adrenal gland. Normally, they either mature or gradually disappear. Others, which neither mature nor disappear, form tumors.

Complications:
A number of problems may result out of neuroblastoma, including:

  1. Metastasis or spread of the cancerous cells
  2. Spinal cord compression
  3. Paraneoplastic symptoms, such as rapid eye movement or difficulty with eye coordination
  4. Abdominal swelling
  5. Diarrhea

Treatment:
Depending on the child's age, the stage of the cancer, or the types of cells involved, the treatment plan varies accordingly:

  1. Surgery - However, this depends on the size and location of the tumor. Tumors growing near vital organs are too risky to remove
  2. Chemotherapy - involves the use of chemotherapy drugs
  3. Radiation therapy - involves high energy beams, like X-rays
  4. Stem cell transplant or autologous stem cell transplant in case of high-risk neuroblastoma
  5. Immunotherapy - involves drugs which stimulate the immune system to destroy cancerous cells

Capcitabine tab cause nausea and vomiting so he must take adequate antiemetics as premedication. Anemia can be due to bleeding from the tumour. Please find out if he is passing black, sticky stool. Chemotherapy also can cause drop all components of blood. Put him on parenteral nutrition for a while so that he is not nutrionally depleted.

As rightly advised by the other doctor, you need to take proper precautions before taking the blood sample. If its persistently high, then you need further evaluation. The symptoms you are having is because of increased prolactin levels in your body. The cause can be a small tumour in the brain, certain kinds of medications etc. An endocrinologist will help you with medicines to control the level of prolactin. If there is a treatable cause, then that should be treated by surgery.
