I am a 65 years old diabetic person having controlled sugar through regular medications. For last one month I have been experiencing swelling in both the legs and pain in the lower leg area normally covered under socks. My doctor has administered 3-days dytor plus and further changed triglucored forte withzoryl-2mg and istamet. Though my swelling in the legs has gone but the pain still remains in the lower leg area which is hardened. Kindly suggest what medications can be taken for the pain and softening the hardened area of lower leg. I am no more using socks for last one month as suggested by the doctor.

I want to know a few more details like your fasting and pp sugar levels, hba1c and lipid profile. Is the pain aggravated by walking and does it reduce on stopping or resting for some time? if it happens that way, please get your blood supply to the limbs checked by colour doppler tests which show the internal blood supply. Because in diabetes peripheral blood vessels are affected by cholesterol deposition. You may also consult a vascular surgeon for this. How is your foot does it feel cold, or shiny looking? please inform me. Thanks.

I am 25, my left knee seems to be a kind of weird. Sometimes I get sudden pain during walking. If I'm running or walking fast, it gives some sudden pain and movement near the knee. Movement meaning, it feels as if the knee is going to break. I had this pain since a year. Earlier it was normal, but these days its getting often..Can you help me out here?

Hello, your problem is may be due to following reasons- 1. Overweight, 2. Deficiency of vitamin d3 or calcium, or lack of water in the body, 3. Postural stress, 4. Muscle strain, 5. Arthritis etc. Please send me all investigation reports if available. Till then you can follow these advises:- 1. Take calcium supplements and sun bath daily to prevent further damage to your joints. 2. Avoid heavy lifting. 3. Avoid squatting and weight bearing activities. 4. Use western toilet. 5. Wear shock absorbing, good quality of shoes while walking. 6. Keep roll of a towel under your knees while sleeping. 7. Perform some gentle exercises and stretching of your all joints before getting out of your bed. 8. Walk straight with shoulder protracted. 9. Hi,first of all you should know what is backache and by which factors it caused and aggravated. The usual cause of low back pain is the sedentary lifestyle that we lead these days. There is hardly any movement. We remain seated most of the time and stand for very short periods of time. Unfortunately walking, running and cycling like activities are also not a part of our daily routine. To add to it, our diet consists more and more of junk food. Fresh fruits, vegetables and high fibre foods are consumed less and less. These daily practices lead to obesity. The increased body weight puts lot of pressure on the spine which has to bear all the extra unwanted weight. The back muscles which are already weak due to lack of exercise and wrong posture are unable to cope with this increased load. This causes pain in the back muscles. Do avoid raising head too high if required do with eyes closed do take ift and ust for 12 days followed by strengthening exercise from neuro physiotherapist best wishes.

If you have a complete tear - then surgery is the only option as it will prevent early osteoarthritis of the knee joint and other complications.

I am an 18year old female, and I have cervical spondylosis for the last 2years. Can you suggest some exercises for me?

The best exercises are those involving stretching the neck backwards, as that works against cervical spondylosis. In yoga, start doing bhujangasana, tadasana, gomukhasana etc. Don't use pillows while sleeping. Put cow ghee 2 drops in nose daily. Massage with medicated oil followed by fomentation. This will relax the muscles. If it is more, then go for greeva vasti at a panchakarma centre.

Osteoarthritis - A Detailed Guide for Treating it!

Osteoarthritis is a Non Inflammatory disease that affects many joints, knee joint most common. In normal joint, a firm, rubbery material called cartilage covers the end of each bone. Cartilage provides a smooth, gliding surface for joint motion and acts as a cushion between the bones. In Osteoarthritis, the cartilage breaks down, causing pain, swelling and problems moving the joint. This disease, that mostly affects women, worsens over the time and should be treated as early as possible.

Symptoms Of Osteoarthritis
Symptoms of osteoarthritis vary, depending on which joints are affected and how severely they are affected. However, the most common symptoms are pain and stiffness, particularly first thing in the morning or after resting. Affected joints may get swollen, especially after extended activity. These symptoms tend to build over time rather than show up suddenly. Some of the common symptoms include:

  1. Sore or stiff joints – after inactivity or overuse
  2. Limited range of motion or stiffness that goes away after movement
  3. Clicking or cracking sound when joint bends
  4. Mild swelling around a joint
  5. Pain that is worse after activity or toward the end of the day

Treatment Options For Osteoarthritis

Osteoarthritis is a chronic (long-term) disease. There is no cure, but treatments are available to manage symptoms. Long-term management of the disease will include several factors:

  1. Managing symptoms, such as pain, stiffness and swelling
  2. Improving joint mobility and flexibility
  3. Maintaining a healthy weight
  4. Getting enough of exercise

Physical Activity: One of the most beneficial ways to manage OA is to get moving. While it may be hard to think of exercise when the joints hurt, moving is considered an important part of the treatment plan. Simple activities like walking around the neighborhood or taking a fun, easy exercise class can reduce pain and help maintain (or attain) a healthy weight. Strengthening exercises build muscles around OA-affected joints, easing the burden on those joints and reducing pain. Aerobic exercise helps to improve stamina and energy levels and also help to reduce excess weight. Talk to a doctor before starting an exercise program.

Weight Management: Excess weight adds additional stress to weight-bearing joints, such as the hips, knees, feet and back. Losing weight can help people with OA reduce pain and limit further joint damage. The basic rule for losing weight is to eat fewer calories and increase physical activity.

Stretching: Slow, gentle stretching of joints may improve flexibility, lessen stiffness and reduce pain. Exercises such as is great ways to manage stiffness.

Braces: For varus and valgus deformity of knee joint, brace very helpful like Unloader Knee Brace has importent role in Osteoarthritis knee.

Management: Medical:
Glucosamine/Chondroitin- Glucosamine/Chondroitin 1500 mg/1200 mg daily, they are diet supplement used in mild and moderate osteoarthritis knee. Major component of joint cartilage. Supplements are derived from the shells of shellfish (such as shrimp, lobster and crab) or from vegetable sources. Slows deterioration of cartilage, relieves osteoarthritis(OA) pain and improves joint mobility.

Intraarticular Injections

  1. Intraarticular Glucocorticoids: Mainly Indicated when pain persist despite of pain killer. Its effective for short-term pain relief < 12 wks. There may be acute flare in pain 48 hrs post-injection.
  2. Hyaluronans (e.g. Synvisc): Its indicated when pain persists despite other agents. Its like Synthetic joint fluid. Pain relief similar to steroid injections. 60-70% patients respond well , pain relief approx 6 months to 1 year

Surgical Management

Knee pain or functional status has failed to improve with non-operative management

Types of Surgery

  1. Arthroscopic Irrigation
  2. Arthroscopic Debridement
  3. High Tibial Osteotomy
  4. Partial Knee Arthroplasty
  5. Total Knee Arthroplasty

Which type of surgery you require it will be decide by your orthopaedic surgeon after clinical and radiological examination.

Joint Replacement Surgery: Joint surgery can repair or replace severely damaged joints, especially hips or knees. A doctor will refer an eligible patient to an orthopaedic surgeon to perform the procedure.
Knee replacement has become the most common form of joint replacement surgery. Between 1991 and 2010, the incidence of knee replacements increased by over 161%, due partly to improvements in the science of knee replacement implants, experience of surgeons, and the growing population of knee replacement patients. Although public perception can be influenced by negative advertising and press, the fact is that knee replacement surgery is one of the most successful surgical procedures performed today.

Success rates and quality of life improvements for knee replacement patients are reliably very high. Nine out of 10 knee replacement patients experience an immediate relief from knee pain, and 95% report they are satisfied with their procedure. Approximately 90% of replacement knee joints last 10 years, while 80% are good for 20+ years, depending on patient health and activity levels. Outcomes vary from patient to patient depend upon level of deformity and grade of Osteoarthritis.

Positive Attitude: Many studies have demonstrated that a positive outlook can boost the immune system and increase a person's ability to handle pain.

Preventive Measures

  1. People with endocrine disorders are prone to osteoarthritis. This includes hypothyroidism. You must immediately begin medications if you are diagnosed with thyroid conditions.
  2. In several cases, osteoporosis is genetic. One may inherit a tendency to develop bone deformities as he/she ages.
  3. X-ray after regular intervals helps the doctors to understand the condition of your bones and joints. This helps to prescribe exercises, medications and diet accordingly.
  4. Yoga helps to maintain joint flexibility and increase bone strength. It also helps to prevent stress and anxiety.
  5. A diet rich in protein and calcium helps to prevent degeneration and brittleness in bones. It is important to go out into the sun once in a while and soak up as much vitamin D as possible. This is a very healthy habit and contributes to the well-being of the entire skeletal systems.

Initially just keep your legs in luke warm water with salt added in it for atleast 15-20 mins. You will feel better and you need to reduce your weight. As it tends to put more load on weight-bearing joints as well as not good for diabetic patients too. And then if it still persists then consult a homeopath for further medications. Take care.

Pain is more likely in muscles. Had there been bony pain. You could not hv moved at all. Cud be a sudden muscle strain. Try dry heating fr 15 mins both sides twice a day fr 3 days. Should subside.

Dentify the mid point between the ankle bone and tendons and give pressure with a blunt edge on either side of your foot for 2 minutes. Also give pressure in the web between 2nd and 3rd finger from your toe for 2 minutes. Finally give a good massage for both feet in hot water. Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Please swing 20 times morning and evenings. This will relieve your problem. Consult after 10 days.

Getting up late in morning, no will to excise regularly because of muscle sprain frequently, too allergic to even things like sweating, pain in back of eyes, frequent headaches, never physically active though try to be, pain in shoulders and joins, had pox for 3 times at childhood in 3 years. What are these symptoms leads to? I feel relieved with paracetamol when prescribed by doctor for any illness. Feel much relieved in brain and muscles. I don't use medicines much unless it is required. Am I need medication or natural remedies are possible?

Hi lybrate-user, I appreciate you seeking help. Well, your symptoms including lethargy & fatigue could be owing to many reasons which needs to be evaluated before recommending anything in specific. Nevertheless, you can try the following in general, and see if it helps - (a) Have a balanced diet with more of leafy greens, and fresh fruits & nuts. Consume fruits/ vegetables rich in vitamin A & C and include more of fresh turmeric in diet on a day to day basis. Among vegetables, onion, garlic, bitter gourd, carrot, beetroot, spinach etc all are to be included too. 'Amla' fruit helps improve immunity and protects against infections. (b) Restrict flesh intake to more of fish and lean meat, and less of red meat, if any. If you are a pure vegetarian, you may take omega-3 fatty acids, 1 cap thrice daily, post meals, for a month. (c) Stay well hydrated - take at least 8 - 10 glasses of clean filtered water daily. (d) Streamline your occupation, reduce stress at work/ occupational stress and stay relaxed. Do yoga and meditate for 15 - 20 minutes daily. 'Yogasanas' & breath control exercises including 'Anulom Vilom' 'Bramari. & 'Savasana' could be helpful. (e) Try exercising daily for at least 30 minutes, walking being the safest. Should the above-mentioned not help still, it is advisable that you consult an ENT specialist who can clinically examine you to ascertain if you have sinusitis. Knee pain is a common complaint that affects people of all ages. Knee pain may be the result of an injury or some other medical conditions including arthritis, patellar tendinitis, acl injury, knee bursitis, torn meniscus, gout and infections. Try some home remedies for your pain- - ice is useful for the control of pain and swelling. It can be applied to the knee for 15 to 20 minutes as often as every two to four hours. The swollen knee should be elevated above the level of the heart while icing. -activities that cause pain should be avoided temporarily. -if the knee is swollen or sore, the following positions and activities should be avoided until knee pain and swelling resolve: squatting kneeling twisting and pivoting jogging aerobics, dancing playing sport specific exercises will help you improve range of motion, strength, and endurance. It is especially important to focus on strengthening and stretching your quadriceps since these muscles are the main stabilizers of your kneecap. Core exercises may also be recommended to strengthen the muscles in your abdomen and lower back. Also maintaining a healthy body weight to avoid overstressing your knees are very important. -a full history of your problem should be taken and your movements assessed by a physiotherapist.
