Knee pain is a common complaint that affects people of all ages. Knee pain may be the result of an injury or some other medical conditions including arthritis, patellar tendinitis, acl injury, knee bursitis, torn meniscus, gout and infections. Try some home remedies for your pain- - ice is useful for the control of pain and swelling. It can be applied to the knee for 15 to 20 minutes as often as every two to four hours. The swollen knee should be elevated above the level of the heart while icing. -activities that cause pain should be avoided temporarily. -if the knee is swollen or sore, the following positions and activities should be avoided until knee pain and swelling resolve: squatting kneeling twisting and pivoting jogging aerobics, dancing playing sport specific exercises will help you improve range of motion, strength, and endurance. It is especially important to focus on strengthening and stretching your quadriceps since these muscles are the main stabilizers of your kneecap. Core exercises may also be recommended to strengthen the muscles in your abdomen and lower back. Also maintaining a healthy body weight to avoid overstressing your knees are very important. -a full history of your problem should be taken and your movements assessed by a physiotherapist. Than consult an orthopaedic in your town who wil examin you and advice x ray and mri. And if there is any ligament injury you need to undergo a surgery.

Some times due to deficiency if vit b12 its happens and also diabetes pt. So keep continue and tk physiotherapy treatment and quadriceps exercise best for you.

It' s osteoarthritis of your knee joint. Get x rays done. Both knees ap view single leg weight bearing. Both legs lateral view. Skyline view for patella both knees. These are three x rays of each leg, total 6 x rays. Review with reports.

Yes it is definately possible to control it by exercises. Osteoarthirits is a degenrative disorder. It can be controlled by professionaly designed physiotherapeutic exercises by restoring the balance between all the muscles around the knee thus preventing erosion of the joints. Exercises will prevent its speedy deterioration and improve quality of life. And yes painkillers are not the solution.

Hello Do proper rest & do hot fomentation for 15-20 min thrice in a day .Take UST followed by IFT for 15 days. Learn strengthening & stretching exercises from physiotherapist. This will help in reducing the pain. Thanks.

Hi i Am 42+. Suffering from right knee pain for past one week to such an extent that am limping. There is also a small swelling on the right side of my knee. Am taking homeopathic medicine and pain killers also applying joint pain relief oil. But still pain is persisting with some amount of stiffness in this knee. Please suggest what should I do. Can you recall any initiating factor for this pain like any twisting injury etc? apart from swelling, is there any associated warmth, skin redness etc? are you having any fever or any associated complaints you would like to mention? you can do some intensive ice packs, use crepe bandage for compression analgesic as per your body' s suitability for 2-3 days limb elevation over pillows for 3-4 days if your pain or swelling increase, then we will have to investigate you further in form of x rays and investigations do not hesitate to contact me if you need any further assistance. Quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day.

Multimodal Pain Relief After Total Knee Replacement

Multimodal Pain Relief After Total Knee Replacement

Knee replacement can be extremely painful. Previously, opioids or narcotics were administered for pain relief. But excessive addition of opioids is not exactly effective for controlling pain.

Multimodal pain management has become an important part of the perioperative care of patients undergoing total joint replacement. The principle of multimodal therapy is to use interventions that target several different steps of the pain pathway, allowing more effective pain control with fewer side effects. Many different protocols have shown clinical benefit. The goal of this review is to provide a concise overview of the principles and results of multimodal pain management regimens as a practical guide for the management of joint arthroplasty patients.

Multimodal denotes administering two or more than two types of medications that work with different mechanisms. The following are the techniques used:

  1. Pre-operative Femoral Nerve Block: Prior to the surgery, a catheter is placed beside the femoral nerve for blocking it. This nerve is located in the upper thigh. Medication is delivered through the catheter for the nerve to be numbed for 24 hours. Thus, pain signals to the brain are blocked. This method reduces the use of narcotics and the consequent side effects.

  2. Patient Controlled Analgesia (PCA): This method is also known as ‘Pain Pump’. An intravenous pump is used to administer pain relief medications, such as oxymorphone or morphine, after the surgery. The control button of the machine could be pressed, by the patient for 6 to 10 times per hour. The machine is used for two post-operative days.

  3. Oral Medications: The oral medications include Non-Steroidal Anti-Inflammatory drugs or NSAID; such as Celebrex which is similar to aspirin, structurally. Alternatively, acetaminophen, such as Tylenol or its equivalent composition, can also be used.

  4. Acetaminophen: It acts on the Central Prostaglandin Synthesis and relieves the patient of pain through multiple mechanisms.

  5. Epidural Analgesia: It produces lower pain scores and involves less time for achieving physical therapy goals. However, this is subject to side effects such as dizziness, urinary retention and itchiness.

  6. Gabapentinoids: These medications include membrane stabilizers such as Gabapentin and Pregabalin.

The objective of multimodal treatments is to provide quick relief to the patient and immediately so. Earlier the rehabilitation, more successful will be the knee replacement surgery. If you wish to discuss about any specific problem, you can consult an orthopedist.
