Knee Pain

Chondromalacia patella (patellofemoral syndrome)

Chondromalacia patella facts
chondromalacia patella is the most common cause of chronic knee pain.
chondromalacia patella has also been called patellofemoral syndrome.
the pain of chondromalacia patella is aggravated by activity or prolonged sitting with bent knees.
abnormal 'tracking' allows the kneecap (patella) to grate over the lower end of the thighbone (femur), causing chronic inflammation and pain.
treatment involves improving the alignment of the patella during contraction of the thigh muscle.

What is the chondromalacia patella?

Chondromalacia patella is abnormal softening of the cartilage of the underside the kneecap (patella). It is a cause of pain in the front of the knee (anterior knee pain). Chondromalacia patella is one of the most common causes of chronic knee pain. Chondromalacia patella results from degeneration of cartilage due to poor alignment of the kneecap (patella) as it slides over the lower end of the thighbone (femur). This process is sometimes referred to as a patellofemoral syndrome.

What causes chondromalacia patella?
The patella (kneecap) is normally pulled over the end of the femur in a straight line by the quadriceps (thigh) muscle. Patients with chondromalacia patella frequently have abnormal patellar' tracking' toward the lateral (outer) side of the femur. This slightly off-kilter pathway allows the undersurface of the patella to grate along the femur, causing chronic inflammation and pain. Certain individuals are predisposed to develop chondromalacia patella: females, knock-kneed or flat-footed runners, or those with an unusually shaped patella undersurface.

What are the symptoms and signs of chondromalacia patella?
The symptoms of chondromalacia patella are generally a vague discomfort of the inner front of the knee, aggravated by activity (running, jumping, climbing or descending stairs) or by prolonged sitting with knees in a moderately bent position (the so-called' theater sign' of pain upon arising from a desk or theater seat). Some patients may also have a vague sense of 'tightness' or 'fullness' in the knee area. Occasionally, if chronic symptoms are ignored, the associated loss of quadriceps (thigh) muscle strength may cause the leg to 'give out' besides an obvious reduction in quadriceps muscle mass, mild swelling of the knee area may occur.

How is chondromalacia patella diagnosed?
Chondromalacia patella is suspected in a person with anterior knee pain, especially in teenage females or young adults. With manual compression of the kneecap, while the quadriceps muscle is tightened, there can be pain. This is referred to as the positive 'shrug' sign. Generally, there is no associated swelling (knee joint effusion).

X-rays or MRIs may be done to confirm the inflammation on the posterior part of the patella.

I have used most of anti dandruff shampoo but still I am suffering from dandruff problem. Sometimes it disappear for few days and suddenly its back what should I do?

BHMS
Homeopath, Faridabad
Hello,Dandruff causes hair loss so its necessary to treat dandruff first. Take Thuja 200, 5 drops in the morning, once in a week. Take Arnica Q and Cochleria arom. Q in equal amount, mix with equal amount of coconut oil, and apply this on your scalp, thrice a day. Wash your hair with B&T Anti- Dandruff Shampoo. Revert me after 1 month. Home remedies are as follows:- Coconut oil helps eliminate dandruff due to its antifungal properties. It also moisturises dry scalp and provides relief from itching. Take some coconut oil and mix half the amount of lemon juice in it. 1. Rub it on your scalp and massage for a few minutes. 2. Wash your hair after at least 20 minutes. 3. Follow this remedy two to three times a week.

I do started lunges exercises from past one month and I don't do it continuously and sometimes I feel very much pain in thighs and is there any side effects with this lunges exercises?

Fellowship in Neurological Rehabilitation, BPTh/BPT
Physiotherapist, Gurgaon
Hi, Good initiative towards fitness! Its normal to have little pain after lunges but be regular. It might be your technique of lunge is wrong or you are not doing proper stretching and warm before doing lunges. There are no side effects of lunges if done correctly so I am suggesting some pointers. 1) while performing a lunge try to keep your knees aligned over your second toe so that the knee is moving in the same direction as the ankle joint. However, in reality we often find the knee translating (moving) forward to the toes or beyond in a squat or lunge movement. The reason for this can be attributed to the length of leg bones (tibia/fibula or femur). 2) During lunge or squat movements, we should always emphasize beginning the movement by pushing the hips backwards before they lower towards the floor. 3) In some instances we’ve seen trainers recommend keeping the back as vertical as possible which is problematic. 4) Do proper stretching exercises before doing lunges and squats. 5) Watch your technique in the mirror, the next time you lunge. Place your hands on your hips or in the small of your back and perform your lunge. If you notice any forward tilting in your hips or an increase in the curvature of your low back, you are compromising lumbar stability. Try this pointer and improve your exercise technique. If still pain persists, check with orthopedician or physiotherapist or follow up with me. Thanks.

I am 37 years old. I had joint problem in my right leg knee, there is little bit swelling and I can't fold that leg more than 5 -10 minutes. Can I know whats the problem occuring there?

Diploma in Acupuncture, Pranic Healing, MD - Acupuncture

Try this for joint pains try this. Identify the mid point between the ankle bone and tendons and give pressure with a blunt edge on either side of your foot for 2 minutes. Also give pressure in the web between 2nd and 3rd finger from your toe for 2 minutes. Finally give a good massage for both feet in hot water. Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Please swing 20 times morning and evenings. This will relieve your problem. Consult after 10 days.

I am 65 yrs old and an accident one year before. There were fractures in my pelvic bone. I was advised bed rest and physiotherapy. But now since last two months I am unable to sit at one place not more than half an hour. All my muscles are paining. I get tingling in the morning. Also suffering from fever which is 99 to 100, my routine is disturbed. I have done blood tests and on vit b 12 and d 3 injections. Almost now more than a month but no improvement. Kindly suggest what must be the reason and what medication I should take.

DNB, MBBS
Orthopedist, Delhi
If u have fever, we have to find the cause of fever and site. Get hb, tlc, dlc, esr and pcr for tb and uric acid levels (blood tests). X rays of the painful joint will have to be done. Treatment will depend on the cause of fever. Get your bone density 3 sites. Done too, maybe pain is due to weak bones. The above scan will be done when you' re off calcium for a minimum of 3 days. Vitamin d and b12 supplementation will continue.
