Pacemaker For Heart Failure And Its Implications

DM (CARDIOLOGY), MD (MEDICINE), MBBS
Cardiologist, Gurgaon
Pacemaker For Heart Failure And Its Implications

The treatment of heart disease has come a long way and one of the more important and fairly common devices used to correct rhythmic problems of the heart is a pacemaker. A pacemaker is a device which is put inside the chest or sometimes even the abdomen to send electrical pulses to the heart to keep it beating at a normal rate.

Pacemaker for heart failure

However, the implications in case of heart failure make the usage of the pacemaker much more complicated. In such cases, the device used is a special type of pacemaker which is also called a bi-ventricular pacemaker. This device sends electrical signals to pump the ventricles of the heart so that they can pump at the same time.

There are many factors to be aware of before undergoing this treatment. Some of the factors to consider are mentioned below.

  • Requirement for a pacemaker in case of heart failure - When a patient has heart failure, their ventricles or lower chambers of the heart aren't able to pump enough blood into it. This could be due to physical problems with the heart or problems with the electrical system within it which cannot function normally. The doctor will perform multiple tests to determine whether a patient is the right candidate for a pacemaker for heart failure.
  • The procedure - In most cases the device is fitted into the chest and it is done without an open heart surgery. Precautions and post operative care for a minor surgery should be followed. The doctor makes a minor incision on the chest and then inserts the device along with three leads which are connected to the ventricles to ensure they keep pumping properly. Sometimes another device known as the ICD or the implantable cardioverter-defibrillator may be put in to shock the heart back to a normal rhythm in extreme cases where the change of sudden death from high heart rate is a possibility.
  • Immediate post operative care - As mentioned above, the surgery involved in putting a pacemaker is a minor one and the patient is usually discharged in next 2-3 days or may be more depending on the comorbidities & complexity of the scenario. However, in most cases, people can go home the next day fairly easily although some monitoring is done for the next few days with daily reporting.

Hypertension During Pregnancy - Techniques for Managing it!

MICOG, MS, MBBS
Gynaecologist, Delhi
Hypertension During Pregnancy - Techniques for Managing it!

Hypertension during pregnancy can be a problem for both baby and the mother. Thus it is of utmost importance that during pregnancy a good health should be maintained along with controlled blood pressure and cholesterol levels. With an increase in multiple births and women of older age the risk of hypertension during pregnancy has increased. But if proper care is taken it can be avoided.

Types of Pregnancy Hypertension:
There are three prominent forms of hypertension that can be seen during pregnancy. The pregnant ladies should be aware of the same. These are:

  1. Preeclampsia: This is the most common and serious hypertension during pregnancy. This hypertension can only be controlled by delivering the fetus, which usually involves complications like death of the mother or child. This occurs 20 weeks after pregnancy.
  2. Gestational hypertension: This form is only prevalent during pregnancy and is not a problem for the mother or baby after delivery. This usually occurs in the last leg of the pregnancy
  3. Chronic hypertension: This form forms either prior to the pregnancy or before 20 weeks of the pregnancy.

Management of Pregnancy Hypertension:
Hypertension during pregnancy can be handled by the following:

  1. In case of severe hypertension, blood pressure medication should be continued during pregnancy.
  2. If you are on ACE inhibitor type medication, then the medication is changed to one that is even safe for the baby.
  3. Your doctor might like to monitor you daily and can advise hospitalization for a few days
  4. If medication is missed, it might lead to uncontrolled life threatening hypertension. Thus the medication should not be missed at any time.
  5. In case of mild hypertension and absence of other diseases like diabetes and kidney disorders, the doctor might stop the medication or reduce the dose. Also, being off medicine does not cause any problem in mild hypertension.
  6. Irrespective of the hypertension being mild or severe, the prenatal appointments should not be missed, so that the doctor can monitor you and the baby. So that problems, like rise in blood pressure, poor fetal growth, and signs of preeclampsia can be spotted and steps can be taken for the same.
  7. In case there is some form of hypertension present the prenatal visits and lab tests will be more.
  8. Apart from the usual second trimester ultrasound, there will be periodic ultrasounds in the third trimester to monitor the baby's growth and the amniotic fluid.
  9. Also regular fetal tests and Doppler ultrasounds will be done to track the baby's growth.
  10. Lifestyle changes should be made. Salt intake should be limited, fresh food instead of processed food should be consumed.
  11. If blood pressure is high then doctor might ask you to avoid exercise especially if you never did before pregnancy. If you wish to discuss about any specific problem, you can consult a Gynaecologist.

Is Your Heart Missing a Beat Regularly - Could it be Arrhythmia?

MBBS, Master of Surgery - General Surgery, Magistrar Chirurgiae (Cardio-Thoracic Surgery)
Cardiologist, Faridabad
Is Your Heart Missing a Beat Regularly - Could it be Arrhythmia?

Arrhythmia is a disorder which is characterized by improper and irregular heartbeats; be it too slow or too fast. This problem occurs when the electrical impulses that are used to regulate your heartbeat don’t work in the correct manner. Your heart may beat irregularly, too slow or too fast.

Symptoms
Although this disorder does not yield many symptoms, a few noticeable symptoms of arrhythmia are slow heartbeat, or a racing heartbeat, chest pain, breathing difficulties and dizziness. Arrhythmia can lead to inefficient pumping by the heart, leading to bouts of fainting. A severe symptom of this disorder is ventricular fibrillation, wherein the ventricles tend to quiver instead of transporting blood. This symptom can cut off blood supply to the vital organs, which may lead to death.

Causes

  1. Coronary artery disease: In this disease, arteries of the heart can get blocked, leading to irregular heart beat and impaired heart functioning.
  2. A heart attack: An ongoing heart attack usually leads to irregular heartbeats.
  3. High blood pressure: High blood pressure problems make you more vulnerable to this condition.
  4. Impaired functioning of the thyroid glands: An overactive or underactive thyroid gland can lead to heart arrhythmia.
  5. Excessive smoking and alcohol consumption: These habits can cause problems with the blood vessels in the body, thus increasing risks of this particular condition as well as other cardio-vascular diseases.
  6. Stress: Too much stress can lead to high blood pressure; that is again linked to heart arrhythmia.

Treatment
Mild symptoms of arrhythmia do not require treatment. However, complications resulting from it warrant treatment. The various treatment options for this condition are:

  1. Pacemaker: A pacemaker is a device that is used to regulate your heart rhythms. It is placed in the body by a surgical procedure and an insulated wire is put, connecting the heart to the pacemaker. In case of an irregular heartbeat, the pacemaker sends out impulses to correct the improper heartbeat.
  2. Implantable cardioverter-defibrillator: This device is used to prevent a dangerous symptom of heart arrhythmia, a symptom wherein the ventricle quivers. It is similar to a pacemaker as it is also placed near the collarbone. In case it detects an abnormal rhythm, it sends out shocks to reset the heart rhythm.
  3. Maze procedure: This is a surgical procedure wherein incisions are made on the upper heart tissue to create a scar tissue. Scar tissues act as insulators to prevent stray impulses.
  4. Alternative therapies: Alternative procedures such as yoga and meditation can help remedy the disorder.

Hi I am 28 year old man, unmarried.I am having Hypertension and been taking medicines for the last 5 years. Now I am on Prolomet Am 50. My family history has BP written all over it. Can I marry and recently I have also been feeling drowsy and took Sugar test and it was on border range?

MBBS, Dip.Cardiology, Fellowship in Clinical Cardiology(FICC), Fellowship in Echocardiology
Cardiologist, Ghaziabad
Hi I am 28 year old man, unmarried.I am having Hypertension and been taking medicines for the last 5 years. Now I am ...
Yes you can get married. All you need is to understand hypertension. Hi Normal range of blood pressure is between 90/60 to 140/90, anything above or below is is abnormal. Many factors can affect blood pressure, including: 1) The amount of water and salt you have in your body 2) The condition of your kidneys, nervous system, or blood vessels 3) Your hormone levels You are more likely to be told your blood pressure is too high as you get older. This is because your blood vessels become stiffer as you age. When that happens, your blood pressure goes up. High blood pressure increases your chance of having a stroke, heart attack, heart failure, kidney disease, or early death. DASH stands for Dietary Approaches to Stop Hypertension. It is an eating plan that is based on research studies sponsored by the National Heart, Lung, and Blood Institute (NHLBI). These studies showed that DASH lowers high blood pressure and improves levels of cholesterol. This reduces your risk of getting heart disease. The DASH Diet Emphasizes vegetables, fruits, and fat-free or low-fat dairy products. Includes whole grains, fish, poultry, beans, seeds, nuts, and vegetable oils. Limits sodium, sweets, sugary beverages, and red meats. Along with DASH, other lifestyle changes can help lower your blood pressure. They include staying at a healthy weight, exercising, and not smoking. I read your query, by the information I have in hand my advice to you would be 1) Keep a BP daily record for at least one week 2) Basic tests should be one to rule out cardiac / kidney issues 3) unless any recent evaluation has been done I would advice these test ECG ECHO Lipid Profile TSH KFT  Renal doppler  Get back to me with a detailed history and  old / new records for a better individualized advice.

Dear lybrateuser, - It is good that your parameters are normal, the chances of a heart attack reduce drastically if your levels are normal - in future to reduce the chances you have to maintain & periodically check that the parameters are normal - a well balanced diet, regular exercise, less stress, proper sleep habits are the lifestyle components which help you lead a healthy & disease free life.
