Abdominal Pain Should Never Be Neglected!

There are several Causes of abdominal Pain.It is a dangerous symptom and should never be neglected.Abdominal pain can be either Acute pain or it can be Chronic Pain.

Commonest Causes of Abdominal pain we see in day to day practice are:

1) Urinary tract stones

2) Gastroenteritis

3) Acid peptic Disorders, Reflux Esophagitis

4) Appendicitis

5) Cholecystitis

6) Systemic Causes: Sickle cell crisi,Metabolic Causes

7) Referred pain: Myocardial infarction, Pleurisy

8) Hernia: Ventral Hernia, Inguinal Hernia, Umbilical Hernia,

9) Malignancies (Cancer) and Tumours of GI Tract, Urinary tract,Retroperitoneum.

10) Infections: Tuberculous, Mesentric adenitis, Liver abscess,

11) Pelvic Causes ( Torsion Ovarian cyst, Ovarian tumours), torsion Testis,Rutured Ectopic Pregnancy.

12) Pancreatitis (Alcoholic, Gall stone)

13) Intestinal Obstruction

One should visit an Experienced General Surgeon who will first take detailed history, and then do a thorough Clinical Examination.You should properly give all history to the Surgeon including all past diseases( Take with you all old records, Investigation reports),Personal History, Family History, History of addictions if any,Appetite, Bowel, Bladder, Sleep,etc.

If required one may be subjected to any of the following Investigations:

Routine Laboratory tests

USG Abdomen ( Ultrasound)

Xray Abdomen

Sometimes CT Scan or MRI

Endoscopy (Upper GI or Colonoscopy)

Other Specialised investigations

Management:

Either Medical or Surgical:After a definite diagnosis is reached, Many causes are managed by Medications alone. Some of them requires Surgical Intervention, which can be either Immediate / Emergency Surgery or planned (Elective).

Examples of Emergency Surgical interventions:

1) Acute Appendicitis

2) Strangulated Hernia

3) Peritonitis due to any Cause.

4) Severe Hydronephrosis with large obstructing Urinary tract Stone

5) Acute Cholecystitis

6) Ruptured Ectopic Pregnancy

7) Torsion of Ovarian Cyst.

Surgeries of the Abdomen can be Either an Open Surgery or more commonly nowadays by minimal Invasive technique called the Laparoscopic Surgery.

The Surgeon should be left to take a call regarding method of surgery as per his/her experience, expertise and equipment available at the centre and patient condition.Not all surgeries are possible laparoscopically in all patients.

Advantages of Laparoscopic Surgery:

1) Minimal Invasive: Small Key Holes only on the abdominal wall. No big incisions,or injury to the body.Therefore minimal local and systemic inflammtory response.MInimal collateral damage to the body

2) Small scar: Better Cosmesis

3) Faster Recovery and back to routine work and life.

I have paraesophagael hiatus hernia which I come to know one month before thru endoscopy but 4 days before I gone for beriniam x-ray in which hiatus hernia is not found also the paining gone down Please suggest next what should I do next ?

Just take PPI, you shall be alright most of the time.In case of no relief may require further investigations like manometry, ph studies, repeat Barium swallow, etc.

Do not go for traditional procedures which can land up in serious trouble. If the hydrocele is small you can wait. Otherwise it needs surgery to cure it completely.

Usually absorbable stitches are taken which fall off on its own. The skin will be more sensitive for few more days. You can use xylocaine oitment for reducing sensitivity a bit. But gradually the sensitivity will decrease with time.

I had done hydrocele surgery 1 year ago and right testicle is still big but I had joined gym 2 days ago is it hydrocele reappear again? If you did daily gym or it will be normal. I'm in tension please reply .Hydrocele will reappear or not?

Hydrocele if operated properly will not recur. If it was a hernia, on lifting heavy weight there are remote chances of a recurrence. Get your self examined by a General Surgeon.

I am suffering from perianal hematoma. Is there any cure of this disease without performing surgery?
A hematoma if small will get absorbed on its own. You can take sitz bath (sit in a hot water tub, take some analgesics. Do not strain while defecating, can use a mild laxative. If too much symptomatic (pain) or a large size, it will require surgical evacuation which is a small procedure.

It is the scrotal skin which is temperature sensitive so that in cold weather the testicles come closer, whereas in hot environment it hangs. It is nature's mechanism to keep testicles at a cooler temperature from body which helps in spermatogenesis. If you feel any of the testis has become larger, you can self palpate and compare both testes. In case of doubt get yourself examined by a General Surgeon.

Gallbladder usually starts producing stones when its wall is diseased. It may be symptomatic or may have no symptoms. Gradually it can develop inflammation called Cholecystitis when it will cause pain. If the stones slips into the bile duct it can cause complications like obstructive jaundice, pancreatitis and others. If you have multiple gallbladder calculus better to get it operated. Nowadays iit is done mostly Laparoscopically unless there is some diffiiculty. Laparoscopic Surgery is minimally invasive. There are no incisions, less pain and quick recovery. Laparoscopic Cholecystectomy is the Gold standard treatment. It is covered by Medical Insurance.

Hi Sir, I have grown unknown small, closed, painless and itchy bumps on my testicles they grow and disappear them self but are always there. They do not look like warts. I didn't have any type of sex with anyone but as I am 18 years old student I have a very active lifestyle with lots of rigorous physical works due to which I sweat a lot and my underwear are almost always wet which I don't get time to change till next 12 to 24 hours. I also get some wet white powdery material inside my penis skin.

Those are temporary skin lesions Can be Tinea (Fungal) infection. Keep the area as much dry as possible. Can use Clotrimazole dusting powder after bath. The whitish collection under preputial skin in the penis is normal, you can clean it while taking bath.
