Hypothyroidism

Introduction
Hypothyroidism happens when your thyroid gland, located at the front of your neck, doesn't produce enough thyroid hormone (underactive thyroid). There are several types of hypothyroidism. The most common is Hashimoto's thyroiditis, an autoimmune disease where the immune system mistakenly attacks the thyroid gland. The disease affects both sexes and all ages, but is most common in women over age 60. Because the thyroid gland helps regulate your metabolism, low thyroid levels cause your body to slow down and can affect everything from appetite to body temperature. Symptoms can appear over time and can be hard to diagnose. Left untreated, hypothyroidism can cause serious health complications.

Signs and Symptoms

Slow pulse
Fatigue
Hoarse voice, slowed speech
Goiter (caused by swollen thyroid gland)
Sensitivity to cold
Weight gain
Constipation
Dry, scaly, thick, coarse hair
Numbness in fingers or hands
Confusion, depression, dementia
Headaches
Menstrual problems
In children, slowed growth, delayed teething, and slow mental development
What Causes It?

There are different kinds of hypothyroidism with different causes. In Hashimoto's thyroiditis, antibodies in the blood mistakenly attack the thyroid gland and start to destroy it. Post-therapeutic hypothyroidism occurs when treatment for hyperthyroidism leaves the thyroid unable to produce enough thyroid hormone. And hypothyroidism with goiter happens when you don?t get enough iodine in your diet. In the developed world iodine is added to salt so goiter is rare, although it still happens in undeveloped countries.

Your thyroid gland produces two main thyroid hormones, T3 and T4. In addition, the pituitary gland produces thyroid-stimulating hormone (TSH), which helps control how much T3 and T4 the thyroid makes. Your health care provider will draw blood to measure levels TSH. When levels of thyroid hormones are low, your body produces more TSH to increase production of thyroid hormones. Your doctor may also test for levels of T3 or T4.
Natural medicine practitioners may pay particular attention to levels of T3 hormone. T3 is the active form of thyroid hormone. Your thyroid gland makes some T3, but the body also converts T4 into T3. If you are unable to convert T4 to T3, your laboratory tests for T4 may be normal, but you still may have all the symptoms of hypothyroidism. Talk to your doctor about including T3 laboratory tests in the treatment of your hypothyroidism.

Treatment Options

Drug Therapies
Your health care provider will prescribe a synthetic thyroid hormone called levothyroxine (Levothroid, Synthroid, Unithroid) that you will take daily. A natural dessicated thyroid hormone drug, made from the thyroid glands of pigs, is also available by prescription. Your doctor will want to adjust your dose over a period of several weeks, after regular blood tests to check the amount of thyroid hormone in your blood.

Complementary and Alternative Therapies

If you have hypothyroidism, you need conventional medical treatment. Nutrition and herbs can help support conventional treatment, but should not be used by themselves to treat hypothyroidism.
Nutrition and Supplements
Following these nutritional tips may help reduce symptoms:
Eat foods high in B-vitamins and iron, such as whole grains (if no allergy), fresh vegetables, and sea vegetables.
Avoid foods that interfere with thyroid function, including broccoli, cabbage, brussels sprouts, cauliflower, kale, spinach, turnips, soybeans, peanuts, linseed, pine nuts, millet, cassava, and mustard greens.
If you take thyroid hormone medication, talk to your doctor before eating soy products. There is some evidence soy may interfere with the absorption of thyroid hormone.
Taking iron supplements may interfere with the absorption of thyroid hormone medication, so ask your doctor before taking iron.
Eat foods high in antioxidants, including fruits (such as blueberries, cherries, and tomatoes) and vegetables (such as squash and bell pepper).
Avoid alcohol and tobacco. Talk to your doctor before increasing your caffeine intake, as caffeine impacts several conditions and medications.
These supplements may also help:
Omega-3 fatty acids, such as fish oil, to help decrease inflammation and help with immunity. Omega-3 fatty acids may increase the risk of bleeding, especially if you already take blood-thinning medication. Ask your doctor before taking omega-3 fatty acids if you take blood thinners such as warfarin (Coumadin) or if you have a bleeding disorder.
L-tyrosine, 500 mg two to three times daily. The thyroid gland combines tyrosine and iodine to make thyroid hormone. If you are taking prescription thyroid hormone medication, you should never take L-tyrosine without direction from your doctor. Do not take L-tyrosine if you have high blood pressure or have symptoms of mania.
Do not take an iodine supplement unless your doctor tells you to. Iodine is only effective when hypothyroidism is caused by iodine deficiency, which is rare in the developed world. And too much iodine can actually cause hypothyroidism.

Herbs ( though not scientifically proven or dependable for hypothyroidism )

Herbs are a way to strengthen and tone the body's systems. As with any therapy, you should work with your health care provider to get your problem diagnosed before starting any treatment. You may use herbs may as dried extracts (capsules, powders, teas), glycerites (glycerine extracts), or tinctures (alcohol extracts). People with a history of alcoholism should not take tinctures. Unless otherwise indicated, make teas with 1 tsp. Herb per cup of hot water. Steep covered 5 - 10 minutes for leaf or flowers, and 10 - 20 minutes for roots. Drink 2 - 4 cups per day. You may use tinctures singly or in combination as noted.
Few herbs have been studied for treating hypothyroidism. More research is needed.
Coleus (Coleus forskohlii), for low thyroid function. Coleus may interfere with certain medications, including some blood pressure medicines, nitrogylcern, and blood-thinning medications, such as warfarin (Coumadin). Talk to your doctor.
Guggul (Commiphora mukul), for low thyroid support. Guggul may interfere with estrogen, birth control pills, and other medications. Talk with your doctor.
Bladderwrack (Fucus vesiculosus), for low thyroid support. Do not take bladderwrack unless directed by your doctor. Bladderwrack contains iodine. Although lack of iodine can cause hypothyroidism, most cases of hypothyroidism in the developed world are not caused by iodine deficiency. In fact, too much iodine can actually cause hypothyroidism. Bladderwrack may also contain toxic heavy metals.

Following Up
After you start on thyroid hormone replacement therapy, your provider will want you to have frequent checkups to monitor its effectiveness.

My mother is diabetic for last 6 years but this week her sugar level was 485 & she is suffering from cold & cough for last 20 days. Drs have prescribed antibiotics & antidiabetic (glimepiride 4 mg + metformin 1000 mg BD) but still sugar level is 351 and no symptomatic relief in cold & cough, also she is having fever sometimes. She doesn't like food now. Nausea is also happening.

Thank you for the query. As per the details provided, your mother is 51 years of age and a diabetic for 6 years. A sugar level of 485 mg% is definitely undesirable, and in your mother's case it has shot up probably due to her infection (cough +cold). Please get her sugar profile done (fasting and 2 hrs post prandial blood sugar and hba1c). Since she has an infection, it will be difficult to control her sugar without treating the infection. It will be preferable to maintain her sugar between 140-180 mg% at this phase. Since she is having nausea diabetic ketoacidosis (a condition arising due to high sugar levels) must be kept in mind. Glimeperide and metformin are good medications, but since they are long acting drugs, in the background of an infection they may cause hypoglycemia. Sometimes to manage high blood sugar levels, temporarily, short acting insulin may be used. Once the sugar levels are controlled, the oral drugs may be restarted. It is advisable to monitor her sugar levels at home, if possible. In the long term, a 1400 kcal/day, refined carbohydrate and saturated fat restricted diet is preferred. Regards

Only medicines will not help to control it, you should make some changes in your lifestyle like eating meals on time, exercising regularly and eating less refined carbohydrate like maida, pasta, bread, noodles, white rice. Have food rich in fibre like whole grains (wheat, brown rice) nuts, fruits and vegetables (avoid potato, sweet potato, arbi, beetroot) etc.

How You Can Prevent Yourself from Factor that Cause Diabetes

The pervasiveness of such a serious disorder can be attributed to its growing significance. In the past few decades there has been extensive research in the field of medical science to analyze and control the causes and symptoms of diabetes. This health condition, if unchecked, can lead one to life threatening disorder.

What is diabetes?

Diabetes refers to a medical condition where a human body fails to produce the required amount of insulin. Insulin is the hormone that carries glucose to your cells. Insulin accelerates a chemical process that signals cells to receive glucose from the blood. Blood derives glucose by breaking down the food you eat; exercise is considered as an inevitable part of daily life as it helps in deriving energy from food by turning it into glucose and a host of other nutrients. A lack of insulin production leads to an accumulation of sugar in your bloodstream thus making you diabetic. Patients of diabetes suffer from upsetting symptoms. In many cases diabetes is so acute that insulin needs to be injected into a patient's body on a regular basis. Timely diagnosis and reliable medical guidance is important for patients of diabetes.

Prevent yourself from these causes:
1. Genetics could lead to diabetes. If there has been a family history you should be more cautious.

2. The kind of food you eat has a severe impact on your health. Irregular food habits, frequent consumption of junk food increases the chance of insulin resistance and diabetes. Hence, medical practitioners advice healthy eating habits as a preventive health measure.

3. Children these days tend to become couch potatoes as they spend a lot of time watching television. Lack of exercise and subsequent obesity could also trigger diabetes in children and adults.

You may experience the following symptoms

1. Low insulin production or complete lack of insulin can result in drastic weight loss.

2. You can feel exhausted from moderate exertion.

3. You may face vision disorders. Poor eyesight and blurry vision are some of the common symptoms.

4. Patients suffering from diabetes have the urge to urinate frequently.

5. Patients suffering from diabetes experience excessive thirst

6. Sometimes, diabetes patients excessively loose weight

7. Small wounds and cuts in diabetes patients take more time to heal

Thank you for the query. As per the history you have provided, you are hypertensive (BP 130/90 mm of Hg, on Tenormin 25 and Stamlo 5 mg), have hyperlipidemia (raised cholesterol) and non-diabetic. It would be helpful, if you would have posted the reprots of the lipid profile. Itis advisable to maintain your Total cholesterol <200 mg%, LDL <120 mg% and HDL >45 mg%. The Triglyceride level should be <150 mg%. Since you have limited walking capacity, you need to modify on the diet. Reduce the intake of saturated fat (ghee, butter etc) and refined carbohydrate. The Statin (Atorvastatin, Rosuvastatin) group of medications is effective in reducing cholesterol, but should be taken only after consulting your physician. Regards.

Hi, since you are on treatment for hypothyroidism, see that tsh levels are maintained between. 1 to 2 milii units in the first trimester, late in the 2nd 0.3 to 3 milli units and 0.3 to3.5 in the third trimester. If your tsh is higher than 2 milli units now, increase the dose of thyroxine. In fact it is better to have tsh levels below 2 even just before conception. Maintaining above mentioned levels is essential for proper fetal development as well healthy out come of pregnancy. Please take care.

It is not as easy task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (self-monitoring of blood glucose? smbg). Though difficult, this can be done by a glucometer and keep a record. Get an idea of fasting sugar (fbs) and 2 hours after food (ppbs) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ insulin will be needed. For sugar control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. For giving you regular follow up you ask me privately with all details like your daily eating pattern and medicines you take. I assure you a proper guidance.

Mr. lybrate-user, Creatinine and protein excretion increase occurs when kidney function is affected. In long standing diabetes, filtering membrane of the nephron is damaged leading to proteinuria and also increase in creatinine. Consistent excellent blood glucose control over the years is needed to prevent damage to kidney. Apart from strict blood glucose control, protein intake also has to be reduced to 0.6 g per kg per day. Reduce excess physical activity, plus avoid foods more in creatinine. BP should be maintained at normal level. TSH 8.6 mU/L is high, but still falls in sub-clinical stage, so no treatment is advised. You should avoid soy beans, soy containing products, radish, cabbage, cauliflower, broccoli, mustard, peanuts and coffee. But it very important to maintain a very strict blood glucose control like FBS closer to 100 mg, PP 170 to 180 mg & HbA1c%<7. Thanks.

In t2dm there are no antibodies produced against insulin, it is the insulin resistance that is the culprit along with improper production of insulin by beta cells of pancreas. You might a drug like metformin 500 mg twice a day in addition to insulin which would reduce insulin resistance and help insulin action. Plus your strict diet and exercising for more than 1 h a day should continue.
