We in india usually do not treat latent tb, where as in american and european countries they may treat it also. The reason for it are: 1. Our 40% of population is infected with tb and treating 40% of the population is not a practical task. 2. Even if an individual takes treatment for latent tb, the tb disease is so common in the community that person may still again get tb infection from a tb patient in community.

By xray chest we can suspect TB but can not confirm without AFB demonstration in sputum by smear, genexpert mtb/rif or culture for TB mycobacteria. By mantoux test or TB gold blood test we can detect latent TB which mean one is infected with TB bacilli but can not confirm active diaese.

Tb gold is more reliable then mantoux test. These tests only detects infection and not disease. If xray chest, ultrasound abdomen and sputum afb is normal get investigated for other cause.
