Not that everyone who gets constipated or has hard stools gets hemorrhoids (pilles). But most often over straining, badly constipated stools are possible reasons.

Hi.

Tell him to take homoepathic medication - Aesculus Hipp. 30/ thrice daily and apply Topi-Aesculus cream over the piles for few months.

If it’s better, then continue or else will have to go for other options such as rubber band ligation, surgery, cauterization, etc.

Management:
