Hello, as you mentioned the symptoms. It is grade 4 with complication of venous ulcer. She need work up and form of intervention to treat varicose vein. Also regular dressing of ulcer.

Best would be to do calf pumping exercises ,and wear compression stockings and walk regularly on an even surface ,don't stand for long at one place for more than half an hour keep moving.

If you have cvi there are 3 modalities for therapy, compression stockings of grade iii are a must for a long duration to avoid any swelling, limb elevation and fluid restriction can be helpful, taking anti coagulants as prophylaxis is indicated, however using aspirin and clopidogrel for cvi is not effective, warfarin was earlier prescribed but with safe anti coagulants like xarelto 15 mg od no need for above medications.
