Hello, there is a recurrence of fibroid. You can be placed on low dose oc pills if periods desired or on continuous progesterone therapy for stopping menses to control fibroid.

First because of age understand and accept higher risk to mother and baby both by discussing same with gynecologist.
Any couple desirous of pregnancy and not getting same naturally must meet gynecologist or infertility specialist accepting facts that it needs many reports and different trials of treatment. This means couple must have patience and go to one in whom they have trust to stick for a long time.
In view of age go for treatment giving fast results like test tube baby (ivf)

The management will depend upon the size of the fibroid. If the current treatment which is progesterone you are taking for endometriosis is causing raised liver enzymes then you should stop it. Liver enzymes will revert back to normal.

Hello, yes the pain and heavy bleeding is most likely because of the fibroids. However following menopause fibroids usually shrink and stop producing these symptoms. Since your wife is already 47 years old. My advice is to manage the symptoms with medications for as long as possible. If not subsiding then need to consider surgery
