If your fibroid is very big and your bleeding can not be controlled with medicines, then you might need a myomectomy i.e removal of fibroid. First try a conservative treatment with medicines but I think that your gynaecologist is the best to guide you for this problem.

You have not mentioned her symptoms. As she is already 48, and is approaching menopause. So no need to undergo surgery at this age. U can start homeopathy as it offers good results.

Depends on site of fibroid. If it is submucous or even if intramural but distorting the uterine cavity, it needs to be removed otherwise NOT.

where is the fibroid located? Was is found out due to a routine scan, or did you have symptoms like excess bleeding during periods or pain?
A 5 cm fibroid does not need any treatment. And 5 to 4.9 is not a big change in size, and is not due to the medications.
Sevista is the reason for period delay. Stop all medicines, and in 3 months the issues should rectify.

After this surgery there is risk of uterine rupture if you attempt normal delivery. You should not take risks and make up your mind for caesarian section.
