The antibiotic which you had taken must be resistant to that bacteria. Urine culture and sensitivity examination should be done to know which appropriate antibiotic should be administered. Only then you may get rid of the infection.

Avoid oily, spicy, junk food, tea,coffee. Take fresh fruits and vegetables. Drink 3-4 litre of water a day. Take cystone forte 2 tab twice a day after food.

The reports are not visible here but indicates some problem in kidney like nephrotic syndrome thorough history is required for the proper diagnosis like his bp, h/o diabetes, what is his gfr and what was his treatment course over years you can consult me if you want incase of nephrotic syndrome he will be managed with corticosteroids.

No its not normal to have 10-12 pus cell per hpf normal is upto 5 pus cells per hpf, if you have any symptoms I suggest you should go for urine culture and sensitivity.
