So as per your symptoms concerned, are more likely to be urinary tract infection. Do one urine routine and microscopic test, if pus cells present go for urine culture and sensitivity, mean while take tab levoday 500 once daily for 5 days, tab urispas twice daily for 5 days, syp alkasol 20 ml with 1 glass of water twice daily for 7 days. Drink plenty of water.

First of all reduce weight, modify food/ do some mobility/ light exercise/ yog/ pranayam etc if not doing already. Send me psa test report or tell on phone. At 44 age, this seems more so need to be seen particularly. Before you go for all the investigations take above measures and get some simple herbal ayurvedics combos which will try to balance your fatty liver and cholesterol etc. I ll prefer natural way if parameters are not worse.
