Take bruhatyadi kshayam, 3 tasf twice a day before meals in half a glass of luke warm water. Take gokshuradi guggal, 2 tabs twice a day after meals. Stop peanuts, aor any other dry fruits. Continue the the regime for 21 days. Avoid cold water. Good luck

Since it's a long duration you should first go for chest x-ray and sputum for AFB. Once confirm ant chronic illness then only treatment can be initiated. And stop him from taking any medication. If sputum AFB is negative then I would advice him for spirometry.
