All the surgical operations performed on any of the endocrine glands come under the roof of endocrine surgery. Endocrine glands function as hormone secretor and have a major impact on the normal functioning of all the cells of body. The procedure for thyroid surgery
The patient undertaking thyroid surgery is given either general anesthesia or local anesthesia. The surgery starts with making an incision, in front of the neck. This cut is made at about 2 - 3cm above collarbone. Then the muscles are divided so that the thyroid can be cut out. This entire operation is to be carried out with utmost care and attention so that parathyroid glands and voice box are unharmed.

Complexities involved in thyroid surgery

  1. While undergoing a thyroid surgery, chances of experiencing chest infection, infection due to anesthetics, heart problems, blood clotting, cut infections still persist.
  2. If bleeding continues post the surgery, it may result in swollen throat. Normal breathing may also be hampered. If during the operation, the voice box is damaged, it may result in hoarseness of the voice.
  3. Damage to the parathyroid glands may cause decrement in level of calcium in blood.
  4. If the scar tissue grows abnormally, it may result in surgical scar.

Remedies for the above complications

  1. Effects due to the damage to the voice box can be diminished by utilizing speech therapy. An additional surgery of the vocal cords can also be an option.

Thyroid surgery is performed with the partial or complete removal of the thyroid gland when the patient is diagnosed with thyroid cancer, abnormal cyst formation, goitre, hyperthyroidism or painful thyroid nodules. There are 3 types of thyroid surgery which are performed – lobectomy (removal of part of the thyroid gland), subtotal thyroidectomy (characterized by the removal of the whole thyroid gland but presence of some of the thyroid tissues) and total thyroidectomy (removal of the whole thyroid gland and thyroid tissues).

The surgery is performed on the patient under general anaesthesia with a small cut over the skin and the subsequent compression of the tissues and muscles to expose the thyroid gland for operation. The factors on which recovery depends mainly include the age of the patient, the type of surgery performed and the extent to which the cancer has spread.

Here are the reasons because of which thyroid surgery is performed:

  1. Surgery might be performed if there is an abnormal enlargement of the thyroid gland, known as goitre, which obstructs common functions such as chewing foods, breathing and talking because of throat blockage.
  2. Surgery is a go-to respite for people suffering from hyperthyroidism as this disorder is impossible to diagnose by means of medications or radioactive iodine. Hyperthyroidism occurs because of a side-effect of another disorder called Grave’s Disease. This disease makes the antibodies attack the thyroid gland, as a result of which the gland tends to overproduce hormones.
  3. The most common reason for getting a thyroid surgery is the presence of abnormal cysts or nodule formation in the thyroid gland. While most of the nodules are usually non-malignant in nature, some of them can be cancerous. The benign nodules can also cause significant obstruction with normal bodily functions if they are not treated on time.

Risks

Like any other major surgery, thyroid surgery has its own share of risks, which are:

  • Severe bleeding, infection and allergic reactions to anaesthetics.
  • Significant damage to the parathyroid glands (which are responsible for controlling the calcium level of the body) and the laryngeal nerves (situated adjacently to the vocal chords).

These risks can be prevented by prescribed dosage of supplements.

The thyroid may be a small gland but plays a large role in the functioning of the body. It is located just below the voice box. Along with producing hormones that help in the transportation of blood, it also helps regulate metabolism, keeps the organs functioning optimally and helps the body conserve heat. In some cases, when the gland produces excessive hormones or when it develops structural problems, it may become necessary to remove the thyroid gland. 

There are several ways of removing the thyroid gland. The most common amongst these are:

1.  Lobectomy
A lobectomy is performed when only half the thyroid gland is affected. In such a case, the doctor will remove only one of the two lobes. 

2.  Subtotal Thyroidectomy
In such cases, the doctor will remove the thyroid gland but leave behind a small amount of thyroid tissue. This tissue can preserve some thyroid function but hormone supplements are often required to supplement the production of hormones. 

3. Total Thyroidectomy

In cases of thyroid cancer or when swelling and inflammation affects the entire thyroid gland, it may become necessary to remove the whole gland along with all thyroid tissues. 

You will need to be admitted into a hospital for a thyroid removal surgery. Once admitted, meet the surgeon and anesthesiologist to answer any questions you may have about the procedure. It is important not to eat or drink anything after midnight on the night before your surgery. The surgery is performed under anesthesia so you will not feel a thing. Once you are asleep, the surgeon will make an incision in your throat and remove a section or all of the thyroid gland. This procedure can take between 2-3 hours. After the surgery, you will be kept in observation for 24-48 hours.

The surgery can result in a slightly raised scar that can take upto 6 weeks to heal. You can resume normal daily activities a day after the surgery. However, it is advised to wait for a week before undertaking any strenuous activities. Your throat may feel sore for a few days following the surgery. This can be treated with over the counter pain medication. You may also develop hypothyroidism. 
The most major risk of a thyroid removal surgery is an allergic reaction to anesthesia. Other risks of this surgery are damage to the nerves connected to the vocal cords and damage to the glands controlling calcium levels in the body.

Thyroid, a small butterfly-shaped gland is present at the lower frontal region of the neck, right beneath the voice box. It produces hormones which regulate metabolism (the breakdown of food by the body to convert it into energy). It even plays a pivotal role in boosting organ functions as well as in helping the body sustain heat. However, too much of hormone production by the thyroid gland might yield structural problems, for instance, growth of nodules (abnormal tissue growth) or cysts (non-cancerous sac-like structures containing fluid) and swelling. Hence, a thyroid surgery is a must once these problems occur. The surgery, administered with general anesthesia, eliminates the thyroid gland either wholly or partially.

Why is surgery required?

  1. The presence of tumors or nodules on one’s thyroid gland is one of the reasons why one should go for the surgery. Although most of the nodules are benign, few can be pre-cancerous or cancerous too. Also, those benign nodules can spell trouble if they expand in size, thus obstructing the throat. They can be problematic as well if they cause the thyroid gland to overproduce hormones, giving rise to a condition known as hyperthyroidism.
  2. Hyperthyroidism can be corrected through surgery. It is often an outcome of Grave’s disease, an autoimmune disorder wherein the body misidentifies the thyroid gland as a foreign body, thereby creating antibodies to combat it. The thyroid gland gets inflamed in the process, resulting in the overproduction of hormones.
  3. Another reason is the enlargement or swelling up of the thyroid gland, termed as goiter. Similar to large nodules, goiter too can clog the throat, thus interfering with one’s breathing, speaking and eating.

Types of surgeries available

  1. Lobectomy: This procedure calls for the partial removal of the lobes when a nodule or an inflammation affects just half of the thyroid gland.
  2. Subtotal ThyroidectomyHere, a small proportion of the thyroid tissue is left behind even after the elimination of the thyroid gland.

Have you undergone a thyroid removal surgery recently or planning to go for one, then you should know some important things that should be kept in mind. Your health care provider will educate you regarding the activities and exercises that can help in recovering from the surgery faster. What to do right after the surgery?
In the days following the surgery, you are required to take care of the incision area. You may or not be allowed to take a bath depending on the dressing on the wound. You are likely to experience slight swelling around the scar. Even though this is normal, you should report to your doctor without further delay. The scar may start feeling hard three weeks after the surgery, and you may apply an unscented moisturizer to prevent dryness while healing.

When to start daily activities after the thyroid gland removal surgery?
You are free to resume most of the normal activities the day following the surgery. But you should wait for at least 10 days or until your doctor allows you to engage in physically strenuous activities. Your throat is likely to feel sore for a couple of days. You can also take an over-the-counter pain medicine like acetaminophen or ibuprofen for relieving soreness. In case these drugs are not offering desired relief, your doctor may prescribe you to take narcotic pain medicine. It is a common outcome of the thyroid removal surgery to develop hypothyroidism. In case this happens to you, your doctor may recommend you to take some form of levothyroxine for bringing down the hormone levels.

Is there any physical restriction after the thyroid removal surgery?
Most surgeons recommend that patients should limit all sorts of physically strenuous works after the thyroid removal surgery. This is recommended for reducing the risk of postoperative neck hematoma and breaking open of the wound closure. These limitations are temporary and brief and are generally followed by a quick transition to unrestricted activity. You are free to take part in heavy lifting or swimming or other exercises two to three weeks after the surgery.

Is it possible to lead a normal life after the thyroid removal surgery?
Once you have recovered from the effects of thyroid surgery, you can do anything you feel like. Post-surgery your thyroid levels will be monitored through regular follow-ups. If the surgery were carried out for thyroid cancer, your doctor would also check for recurrence of cancer during your visits. It is essential to stick to this schedule and report if you notice any suspicious changes or experience discomfort so that proper care can be given and you can lead a normal life.
