lybrate-user, Thanks for the query.
The recommended TSH levels are as follows
1st Trimester: 0.1 to 2.5 mU/L, 2nd Trimester :0.2 to 3.0 mU/L & 3rd Trimester: 0.3 to 3.0 mU/L. Since you are in the first trimester, TSH should be a bit lower than the present level. So may increase the dose by 12.5 mcg per day. You should check TSH once every 4 weeks during the pregnancy period. Thanks.

lybrate-user, Thanks for the query. I remember to have answered the same query a few days back. Most important is to maintain TSH level around 2 mU/L during the whole pregnancy. That is necessary for proper fetal growth and expected outcome of pregnancy. You will have to check TSH level every 4 to 6 weeks. So my suggestion is to follow the endocrinologist opinion. Thanks.

During pregnancy it is absolutely essential to maintain TSH levels if the following manner: 1st trimester: 0.1 to 2.5 mU/L, 2nd trimester: 0.3 to 3 mU/L & 3rd Trimester: 0.3 to 3.1 mU/L. Your present level is bit too high. It is better to increase Levothyroxine tablets from 62.5 to 87.5 (by 25 mcg per day) and check TSH after 4 to 6 weeks. For proper fetal development and desired outcome of pregnancy TSH should be maintained in the above range. If you have...more

It appears your wife has hyperthyroidism. Radio iodine therapy is one of definitive treatment for this condition. Treatment has no major side effects but pregnancy should be avoided for at least 6 months after iodine therapy and it should not be given during pregnancy.

Presence of ANA positivity suggests autoimmune disorders like SLE which is 10 times more common in females than men. As it is the first screening test. Further detailed investigation is required to make the diagnosis in your patient.
