How To Deal With The Presence Of Foreign Bodies In Ear/Nose/Throat?

What is a foreign body (FB)?
Foreign body or FB is an object or piece of extraneous matter that has entered the body by accident or design. It is common to see certain items getting stuck in the ear, nose or throat especially, in young children. While most of the time they do not raise an alarm, however, sometimes they can lead to severe consequences like rupture of the eardrum or even choking.

How do FBs affect ear, nose and throat?

  • Children over 9 months develop an ability to pick up objects. As they explore the environment around them, they tend to put items such as peas, beans, marbles, beads, cotton, paper, etc, in their mouth, nose or ears. A similar trend can be seen in adults with mental disorders.
  • Trauma/ fall/accident can force FBs like rock, pebbles into the ear, nose or throat.
  • Adults while riding a bike or sleeping on the ground (campsites) can have insects: flies, ants, roaches fly into the nose or ear.
  • Choking occurs when FBs block the airway, common reasons are: not chewing the food properly, diseases like Parkinsonism, drinking alcohol or certain drugs that slowdowns the process of swallowing.

Managing FB in the ear, nose and throat

Foreign bodies in the ear
Symptoms: Pain in the ear, discharge or bleeding from the ear, feeling of fullness or pressure in the ear and dizziness and nausea in extreme cases.

Management:

  • Make sure the patient is sitting in an upright position, pull the pinna or outer semicircle of the ear towards the back. This can help you see the object clearly and also loosen it to wiggle out. If you can see the object clearly, use a tweezer to grasp the object and gently remove.
  • In case a live insect gets trapped inside the ear lie down and place few drops of warm vegetable or baby oil. Use oil only if symptoms of eardrum perforation like pain, bleeding/discharge from the ear is absent.
  • Tilting the head to the affected side can facilitate gravity to dislodge the object.

Foreign bodies in the nose:
Symptoms: 
Pain and difficulty in breathing from that side of the nose.
Management: Keep the uninvolved nostril closed by pressing a finger against the side of the nose and gently ask the person to blow from the affected nostril.

Foreign bodies in the throat:
Symptoms: Choking, inability to breathe and speak 
Management: Perform Heimlich maneuver: abdominal thrusts, if trained otherwise, it is advised to call for medical help immediately.

How can you prevent foreign bodies from entering your ear, nose, throat?

  • Discourage talking, playing, laughing when food is in the mouth.
  • Do not give whole grapes, popcorn, nuts to children below 3 years.
  • Keep small objects out of reach of small children.
  • Teach children to avoid placing objects into their nose, ears.

Tonsillitis is a condition, which is characterized by infection of the tonsils in your throat. The tonsils are a pair of lymph nodes that are situated in the throat; its primary function is to protect the body from various infections. Although the disorder can occur at any age, it is majorly common among children. A large number of bacteria or viruses can cause this disorder, but the most serious symptoms occur when it is caused by the streptococcal bacteria.

Causes

The first defense mechanism of the body is made up of the tonsils. They synthesize white blood cells which fight against infections. Although tonsils combat these infections, at times, they can fall prey to the same, thus resulting in tonsillitis. Tonsillitis can be caused by both virus and bacteria; however, it is mostly caused by the former. Kids and school going children are very vulnerable to this disorder as they can come in contact with harmful viruses and bacteria very easily.

Symptoms

You will experience symptoms such as a sore throat, bad breath and have difficulty in speaking. Swelling of the tonsils will also make swallowing of food a difficult affair. Other symptoms of tonsillitis are stomach cramps, fever and headaches. The neck and the jaw may also feel tender due to the swollen lymph nodes.

Treatment

If the symptoms are mild, then it usually does not require any specific treatment. For severe symptoms, antibiotics are prescribed to combat the bacteria that caused the infection in the first place. Chronic cases of this disorder warrant a surgery known as tonsillectomy. In case of dehydration, intravenous fluids are administered.

In addition to these treatment methods, there are certain home remedies that you can follow. The tips you can follow are:

  1. Keep yourself well hydrated
  2. Use a humidifier
  3. Gargle with lukewarm water mixed with salt, regularly

Sore Throat and Strep Throat - Are They Same?

Throat pain mainly occurs due to two conditions-'sore throat' or 'strep throat'. Often the terms 'sore throat' and 'strep throat' are used interchangeably, but they don't indicate the same thing. While a sore throat is a condition mostly followed by common cold, runny nose or congestion; strep throat is a bacteria-induced infection. The bacterium called Streptococcus is responsible for strep throat.

The most common reason behind a sore throat is a viral infection like flu or cold. So, if you've a sore throat that is caused by virus, there is no reason to worry as it can be treated easily. But if you are suffering from strep throat or streptococcal infection, then antibiotic drugs for additional treatment will be required to avoid complications.

Signs and symptoms of a strep throat (which indicates that your throat pain is serious and needs attention) include:

  • Pain while swallowing food
  • Sore throat which usually persists for more than 48 hours
  • Small red spots at the roof top of your mouth
  • Swollen and red tonsils at times with streaks of pus or white patches
  • Fever
  • Swollen, benign lymph nodes in your neck
  • Headache
  • Rash
  • Body aches
  • Vomiting or feeling of nausea, especially amongst younger children

When these signs and symptoms become noticeable, you need to promptly visit your doctor for test and treatment. And, if the reports indicate strep throat, then your doctor will prescribe you some oral antibiotics to reduce the complications and severity of symptoms so that it doesn't spread to others. You are advised to follow your prescription properly and stay indoors for at least a day after starting your medication.

What Should You Know About Head and Neck Cancer?

Head and neck cancers usually start in the squamous cells and are also referred to as squamous cell carcinomas of the head and neck. It occurs in the nose, throat, mouth and salivary glands. Head and Neck cancers are further classified into various categories on the basis of the area of the neck and head where they occur. Although head and neck cancers are different, they both occur inside the mouth, sinuses, nose, salivary glands and throat, which are considered as a group.

Salivary gland cancers are generally very infrequent. An assortment of cancer causing cells is present in salivary glands, so various types of salivary gland cancers have been found to occur.

Causes

If you are habituated to consuming tobacco and alcohol, then you are at the highest risk of suffering from head and neck cancer; however, it can also be caused in people who suffer from infections caused by human Papilloma virus. Other factors may include:

  1. Oral health- Untreated gum diseases and oral cavity can be the reasons behind the development of the disease.
  2. Sex- Women are less prone to head and neck cancers than men.
  3. Lack of Nutrition- If your body is lacking in certain nutrients; you are more likely to catch the disease.
  4. Age- People above 40 years of age are more susceptible to head and neck cancer.

Symptoms-

  1. Difficulty in swallowing- Inflammation and pain in throat, leading to a problem in swallowing.
  2. Hoarse Voice- Hoarseness or change in the voice.

Apart from this, there are some symptoms which show up in a specific area of your head and neck, such as:

  1. Oral Cavity- A red or white patch on the tongue, the gums or the lining of the mouth.
  2. Pharynx- Trouble in breathing and swallowing followed by a frequent headaches.

Treatment

  1. Surgery- Surgery is usually to remove the primary tumour in toto or as much as feasible. Also, clearence of the lymph nodes that are involved is done during surgery at the same time.
  2. Radiation Therapy- Radiation therapy is considered where functions like speech can be conserved.
  3. Chemotherapy- Radiotherapy in combination with chemotherapy can help, if you are suffering from a more advanced form of head and neck cancers.

Tonsillitis is a disorder of the tonsils present on the back of the throat. 

It is mere * myth * that the tonsils acts as a defensive mechanism in the body as they prevent the body from infections. Tonsillitis can occur at any age. It is widely prevalent among school going kids as they get exposed to the different environment for the first time.

With age advances frequency of attacks reduces because the body gains immunity after each episode.

Tonsillitis may be classified into two types: 

  1. Chronic tonsillitis: when frequency and duration of acute attacks increases. This type of tonsillitis tends to last for an extended duration with symptoms of throat pain fever occasionally ear ache off and on also known as acute on chronic illness
  2. Acute tonsillitis: This is a form of acute infection which usually doesn't recur but may set once or so.
