It may be hydrocele of large testis or else atrophy of smaller one. Get a scrotal sonography done to determine exact diagnosis.

Hello-
It seems that you are suffering from retractile testicle. It is a testicle that may move back and forth between the scrotum and the groin. It is a matter of concern as longer the testicle reside in groin or away from scrotum, sooner it will get damaged (due to body temperature) and fertility can be affected.

Very tricky situation.
If we keep undescended testes in scrotum first it is useless functionally.
Second there are chances of cancer development in it.
But as you have only one testes removal may be undesirable, so you should be under good follow up to detect early malignancy
