I suppose that most of your issues can be ascribed to you being a diabetic and thus can be resolved by discussion with an endocrinologist / general physician who is managing your diabetes. If you have back pain with radiation to legs with associated symptoms than we can think of spine as a cause of your symptoms and then you may get in touch with me. I have seen the details mentioned. The symptoms of peripheral neuropathy like burning pain or irritation, those can be effectively controlled by using certain medications like gabapentine, pragabalin & methylcobalamineetc. But it is difficult to "cure" those. The reason is those are part of the long term complications of diabetes mellitus. Neuropathy tends to be more severe and repetitive if blood glucose is not in proper control. Your diabetes is fairly controlled. But ed means you may have peripheral artery disease. Please first check a lipid profile and serum testosterone levels. Later consult your doctor for further advice. Good luck.
