Lymph node enlarged for 3 years-Need answer of following questions-
1-which lymph nodes-area
2-how many lymph nodes
3-how much enlarged
4-they are painful or not.
5-any change in the size recently.
6-consistency of lymphnodes
7-any associated symptoms.
Giant pletelets may seen in peripheral smear due to many reasons like Because of EDTA/anticoagulant used in sample collection, because of certain antibiotics or in indicate ITP (immune thrombocytopenic purpura)

We need to address the axilla especially for staging. 30% of nodes which are negative for cancer on ultrasound, PET scan and Core Biopsy or FNAC, turn out to be positive on sentinel node Biopsy.
Ideal procedure for a negative axilla should be a sentinel node Biopsy and if found positive, then an Axillary clearance should be done. (some prefer to leave it at that if 1-2 nodes only positive or radiate the axilla)

Most probably reactive enlargement to some infection in the vicinity. Follow your doctor's advice to take short course of antibiotic.

Report has clearly mentioned it ae benign lymphadenopathy without focal lesion locally. Hence do not worry. I think it is CT scan report. If you want more assurance get FNAC test of largest lymph node done.
