Lymph node enlarged for 3 years-Need answer of following questions-
1-which lymph nodes-area
2-how many lymph nodes
3-how much enlarged
4-they are painful or not.
5-any change in the size recently.
6-consistency of lymphnodes
7-any associated symptoms.
Giant pletelets may seen in peripheral smear due to many reasons like Because of EDTA/anticoagulant used in sample collection, because of certain antibiotics or in indicate ITP (immune thrombocytopenic purpura)

We need to address the axilla especially for staging. 30% of nodes which are negative for cancer on ultrasound, PET scan and Core Biopsy or FNAC, turn out to be positive on sentinel node Biopsy.
Ideal procedure for a negative axilla should be a sentinel node Biopsy and if found positive, then an Axillary clearance should be done. (some prefer to leave it at that if 1-2 nodes only positive or radiate the axilla)
