While injuries of any form can be painful, burn injuries are one of the worst to suffer from. While cuts and bruises can be temporarily assuaged with a proper dressing, burns are quite different in this regard. With burns, the damaged tissues will stay on the body and may cause continuous pain. Thus, burn injuries are difficult to treat and they require different strategies for pain management as well.

Treatment methods available for treating pain arising due to burn injuries

  1. Apply an anesthetic cream or aloevera gel to soothe the burnt skin. Avoid exposing the skin to air as that will cause painful sensations.

Osteoarthritis - How Best to Manage it?

The human body is able to move fluidly because there are various joints, muscles, tendons and cartilages that work together to make this happen. Movements of the hands, legs, back, hips, knees and wrists are controlled this way. For various reasons, however, these tissues grow stiff, thereby, reducing free movement. Any movement causes pain and stiffness and swelling of the joints, which forces the patient to reduce physical activity. This reduced activity further leads to the joints becoming stiff, thus putting the patient in a difficult position.

What causes it?

The ends of the bone are covered by softer tissue called cartilage, which is responsible for allowing the free movement. In patients with osteoarthritis, this cartilage is lost and the bones rub against each other, producing pain and a crackling sound with the movement. Some of the most common causes include the following:

  1. Age is the most common cause for osteoarthritis, with lifestyle habits playing a major role in this condition. Most joints would be affected here.
  2. Obesity is another major cause for osteoarthritis, as the additional weight of the person puts extra pressure on the knees and the legs, leading to the wearing of the joints. The back and lower extremities are more affected than the hands in this case.

Charcot foot is a rare complication of diabetes. It usually affects diabetics with peripheral neuropathy. Charcot affects the joints, bones and soft tissues of the ankle or foot. This causes the bones to become brittle and fragile, and the joints to become dislocated. Over time, the foot joints collapse and become deformed.

What causes Charcot foot?

The following conditions/factors may cause Charcot foot to develop in individuals –

  • Spinal cord injury/disease

  • A fracture or sprain that does not heal quickly

  • A footsore that refuses to heal

  • An infection

As these problems start to show up, the patient may not be aware that the foot is hurt because of nerve damage – a complication common in diabetic mellitus patients – that keeps you from feeling or sensing any pain.

What are the symptoms?

Although you might not be able to feel the pain due to loss of sensation, you are likely to notice the following symptoms if you have Charcot foot –

  • Swelling in the ankle or foot

  • The foot/ankle may appear reddish

  • Feels warm to the touch

Diagnosis of Charcot Foot

Treatment/Management

The goal of Charcot foot treatment is to heal fractured bones and prevent deformity and further damage to the joints.

Non-surgical treatment involves –

  • Casting – In the initial stage, a cast boot or cast protects the ankle and foot. It is an effective method of managing the symptoms of Charcot foot. The cast helps reduce swelling and acts as a shield to the bones.

  • Custom Shoes – A custom shoe or diabetic shoe is recommended once the initial swelling has reduced and the bones have fused together. The shoe is specially designed to reduce the risk of foot sores/ulcers.

Surgery is recommended if the foot deformity increases one’s likelihood of getting ulcers, or if the specialized shoe wear is not effective. The surgeon will fuse or align bones to stabilize the foot. He might also level sharp bones that could cut into the skin of your foot and cause ulcers.

There are other ways to manage Charcot foot.

  • Do not put too much weight on the injured foot

  • Wash your feet daily

  • Always wear shoes and socks

  • Get regular checkups with a specialist who treats diabetic foot problems

Follow the doctor’s instructions to ensure the best outcome from treatment. Patients with Charcot foot must inspect both their feet carefully on a daily basis. You should also work to keep your blood sugar levels under control to avoid complications.

A fracture is a complete or incomplete crack, which appears on a bone due to application of intense pressure or force. A fracture results in extreme pain, and the bone becomes immobile. Any kind of movement boosts the pain. Fracture pain occurs in three distinct stages. Acute pain is felt immediately after a fracture while sub acute pain occurs over the weeks that follow after a fracture. The third stage or chronic pain occurs when the fracture and soft tissues around it have healed.

Fracture pain is quite intense in nature and proper precautions should be taken while dealing with them:

  1. Development of fracture pain: When a bone undergoes a fracture, the ligaments and tendons are also damaged. While the bone slowly heals, ligaments and tendons fail to heal equally well or completely. Post fracture pain also develops when the outside of the bone does not heal completely. In cases of fracture pain, which occurs due to the incomplete healing of the outer part of the bone, Prolotherapy helps in strengthening the fibro-osseous junction at the source of the pain. Other measures:

  • Splints are utilized to stop the movement of fractured bone and braces can be used to support the bone. Plaster cast also supports and immobilizes the fractured bone. Traction and surgical implants are other treatment measures.
  • Fracture pain occurs not only because of the broken bone, but also because of the weakened ligaments and tendons around the bone. When a blow or a fall causes an injury, where the top of your arm bone pops out from your shoulder socket, it’s called shoulder dislocation.
