Slip disk is a condition of the spinal column which consists of a stack of vertebras from the cervical, thoracic and the lumbar spine. You need to post your mri images. If there is compression of nerve, you need a decompression procedure of some sort. If you are put on a rehab, follow strictly the protocol and find improvement in 6 weeks and further down the line.

You need to start doing back extension exercises and that to only when pain subsides.
Avoid forward bending and lifting heavy things.
If it doesnot settles on 3 months time I would suggest you to undergo endoscopic discectomy under local anaesthesia.
