Hello Sumit, First of all , keep one thing in mind this is not life threatening. So far you could tell three problems mild liver enlargement, mild spleen enlargement and raised ESR. First two are subjective and can differ depending upon examiner. Raised ESR shows some sort of inflammation which can be of many cause. It is better to consult a Gastroentrologist and get these minor thing clarified as it is difficult to comment on limited history and investigations.

The mean liver size was 150.04 ± 14.84 mm among males, 147.57 ± 18.32 mm among females, and 149.8 ± 16.73 mm in the whole study group.
Normally, the spleen does not extend inferior to the left costal margin; hence a normal spleen is seldom palpable through the anterolateral abdominal wall. It varies in size and shape but it is usually 12 cm long, 5 cm thickness, and 7 cm wide.
As you can see, your size is normal and you need not worry
