Ibs is a condition of poor digestion caused by unhealthy food habits, sedentary lifestyle and largely effects large intestine. In modern medicine there is no cure for it. Only symptomatic relief in gas, bloating, cramps, constipation or lose motions or mix of these off and on. Symptoms go and come back again n again.

Ayurved has the answer. Treatment of'grahani' disease is given in ayurved. It is caused by weakness of'agni' (digestive fire) and imbalance of doshas (vata, pitta, kafa) leading to weakness of other body elements (dhatus).

Treatment by herbs and minerals, which are generally long nature treatment regimes, fetch better results by ayurved. The approach is broadly deepan, paachan and shodhan steps. Its generally confusing to decide but in my experience in ayurved spanning three and half decades, I have seen that above approach is good to begin with. When agni is flared up and digestive enzymes act well and excretions are normalized, parpati treatment is introduced which goes for about 40 days. In my lybrate tips about 2-3 years back I had written about parpati kalp. It is very effective. If food habits and lifestyle is improved, the problem doesn't come back easily. Other herbs such as aegle marmelos, zingiber officinale, psyllium husk and use of wrightia antidysentrica are of great use. Modern medicine might not have found buttermilk useful but ayurved experience strongly favours use of'takra' (buttermilk) with cumin seeds powder and other spices. Nutrition is of course important but its successful only once agni is balanced. By ayurvedic solutions, one may prevent complications of other health issues comfortably.

Constipation is when a person has difficulty passing stool or has very dry stool. With processed and refined food habits on the rise, constipation is also becoming a rampant problem. The food passes through the large intestine, where a large part of the water is reabsorbed and stool is formed. This is expelled from the body, and if there is constipation, this passage of stool can be difficult, painful, and may be accompanied by bleeding too.

Constipation is common in children as well as among the elderly. Children eat a lot more junk compared to adults. With tooth loss and reduced food consumption, elders eat a lot less and so constipation is common.
Treating constipation should be a conscious effort as it does not have a single remedy. A combination of dietary changes including all aspects like fiber, water, caffeine etc. can help in treating constipation effectively. 

  1. As the first thing in the morning, drinking a glass of lukewarm water is good. Add some baking soda to this, if you are feeling bloated, as it aids in cleansing the bowels.
  2. Ginger tea is another great remedy to induce bowel movements in the morning.
  3. A glass of warm milk with a little honey or sugar is a good remedy for constipation.
  4. Warm water or milk with Isabgol husk at bedtime helps in easy passing of stools.
  5. Applying a few drops of lukewarm castor oil over the navel when going to sleep can help in easy passage of stools in the morning.
  6. Guava, pears, oranges, grapes, papaya, apples, and other fruits add roughage to the diet and help in easy passage of stools.
  7. Soak some raisins in hot water, cool it, crush and strain them. This is very effective for treating constipation in children.
  8. A glass of carrot juice or papaya juice is very useful in treating constipation.
  9. Drink about 8 to 10 glasses of water to avoid constipation. The artificial juices and canned drinks do not count in this, as they only make it worse.
  10. High-fat, processed foods, alcohol, and caffeine are major causes of constipation. So one should avoid them to a large extent.
  11. Including small amounts of olive oil, castor oil, and ghee help in lubrication and easy passage of the stools.
  12. Triphala, which is a popular Ayurvedic preparation, can be mixed with warm water and consumed to prevent constipation. Given its bitter taste, you can add a small amount of honey to it. Constipation is the most widely found problem of mankind. Almost the largest number of drugs, are available for the problem and largest sums of money are spent by people to get rid of it. Even then the medical science does not recognise it as a serious disease.

To some extent they are correct. Constipation is not a disease as such. It is merely a condition but it can be responsible for attacks of almost all diseases which are effecting the humans. Unfortunately it is also spreading wildly as the civilization is spreading and eating refined foods is becoming fashionable. Many drugs also cause constipation. If so the prescribing doctor should be immediately contacted for change of the prescription.

The present treatment of the condition is drugs of three different types. A) Lubricating, B) Bulk forming and C) Agitating.

  • Lubricating drugs include various oils, milk of magnesia etc. These drugs lubricate the elementary canal and/or lubricate the faeces and thus facilitate the movement. These are the least damaging type of drugs. Yet they can make even undigested foods to pass through the system or cause diarrhoea.
  • Bulk forming drugs include isabgol, rice bran etc. When the bulk is increased the faeces move and are expelled faster. It should be remembered that these are just substitutes for the fibre which is lost during various processes adopted in kitchens. It is clearly foolishness to first throw away the natural fibre and replace it by purchasing substitutes from the market. Further rejected natural fibre carries away several essential nutrients with them, which are not replaced by the additives.
  • The agitating type drugs include several herbs and chemicals which are not acceptable to the system and therefore the system works extra harder to get rid of them. These ultimately tire out the system and weaken it. The weakened system requires more powerful drugs and the endless race starts. Thus this category causes maximum harm to the digestive system.

It is now obvious that all the above drugs are not the cure for constipation. These have only a one-time effect and can harm the system.

In fact, the cure for constipation lies in lifestyle itself. To get rid of it, one should follow the following advice:

  1. Avoid refined foods.
  2. Eat maximum foods in raw form.
  3. Eat lots of salads and fruits so that there is enough fibre in the food.
  4. Avoid refined flour (Maida), polished rice etc.
  5. Do some regular exercise so that the body, including gut is in good health.
  6. Eat sprouted boiled pulses instead of pulses available in the market.
  7. Do not peel all those vegetables which can be used without peeling. Even if peeling is absolutely necessary do by minimum possible.
  8. Drink sufficient water. (Humans need about 3 litres of water every day).
  9. Try discharge faeces twice a day-Morning and Evening.
  10. Never ignore an urge. Attend to it as early as possible.

Even then sometimes the faeces become dry and hard. In all such cases instead of applying pressure it is better to take a plain water enema (Douche). It is totally harmless, does not form habit, and convenient to take.

It is very important to get rid of constipation. Since constipation is the main cause of several diseases.

Some regular habits are just taken for granted and only when there is a problem do we realize the importance. Passing stool is one such habit. When it does not happen (due to whatever reason), then there is an accumulation of toxins within the body, which can have severe physical effects all over the body.

The anus, through which the stools pass out of the body, is about 5 cm long and is highly vascular. The blood vessels can get swollen and cause what are known as piles or hemorrhoids. They can be dry or wet. When there is no bleeding, they are known as dry piles. When there is too much pressure exerted, these blood vessels can rupture and cause bleeding. This is known as wet piles.

Why piles happen?

Stool formation and passage needs fiber, roughage and adequate water. With changing diet patterns, there is a lot of processed and refined foods. There is less of fiber, roughage and water. Difficulty in passing stools and bleeding are the most common symptoms of it. 

Ayurveda brings a holistic approach to treating piles, including food and lifestyle changes, topical remedies and increasing exercise.

  • The most important way to treat piles is to provide more roughage and fiber so stools are formed and easily passed out of the body. This requires eating more fresh vegetables and fruits and drinking lots of water. In addition, whole grains, dairy products, nuts and fruits should be consumed.
  • Refined foods and processed foods should be completely avoided. Also to be avoided are aerated drinks, sodas, pizzas, pastas, and other junk food.
  • While we do not realize how time flies when we sit at the desk, it can also lead to piles. Take a break every hour or so and reduce the incidence of piles. It improves blood flow and muscle movement too.
  • Get into the habit of passing stool first thing in the morning. It may take some time to get into this habit, but it is one of the best habits to have.
  • When the piles are active, try a sitz bath, where there is warm water in a tub to which herbal oils are added. This provides pain relief, dilates the blood vessels, and helps in treating piles.

Hemorrhoids or piles are diagnosed by swelling of veins in the lower part of anus and rectum. Due to swelling, the walls of rectum and anus are stretched and thus become irritated. Hemorrhoids are of two types: Internal and External hemorrhoids.

Internal Haemorrhoids: As the name suggests, the piles are not visible from outside. These are present deep inside the rectum, located above the pectinate line and these are covered with the cells that are present through the rest of intestines. Internal hemorrhoids are normally painless. The first sign of internal hemorrhoids is rectal bleeding. However, straining can push internalhemorrhoid and it protrudes through the anus and this can be painful.

Causes of Haemorrhoids
The swelling of hemorrhoid occurs only when the pressure in the small vessels of rectum and anus is increased and is engorged with blood. The size of the hemorrhoids is also increased. The increased pressure may be due to various factors:

  • Consuming low fiber diet and pushing or straining while passing stools, thus blood vessel pressure is increased.
  • Pregnancy also is a cause of swelling as there is the increased pressure of the uterus on rectum and anus.
  • Sitting for long on toilet also increases pressure.
  • Obesity is also a cause of hemorrhoids.
  • People suffering from diarrhea, both acute and chronic.
  • A person having colon cancer might have hemorrhoids.

Signs and Symptoms of Haemorrhoids:

  • The most common symptoms of hemorrhoids are painless bleeding, itching in anal, swelling and feeling of the presence of lump at the anus.
  • All these symptoms are associated with inflamed hemorrhoid.
  • Inflamed internal hemorrhoid can cause swelling, but is painless as there are no fibers attached to veins above the pectinate line that cause pain. However, passing hard stools can scrape off the thin lining of hemorrhoid, which can cause the painless bleeding.
  • Spasms of the rectum and anal muscles caused by these swollen hemorrhoidscause pain, especially when they are protruded through the anus.
  • Internal hemorrhoids can form a clot or thrombose resulting in severe pain.
  • The presence of has inflammation, stool,and constant moisture and these can lead to itching and burning sensation in the anus among patient of internal hemorrhoid.

Treatment of Haemorrhoids

  • Diet: Haemorrhoids can be treated when individuals consume a fiber-rich diet in order to soften the stools and ease their bowel movements. Fiber-rich foods include fresh fruits, dry fruits, grains, vegetables, and cereals. About 20 to 30 grams per day of fibrous foods is recommended. Dietary fiber will help create bulk in the intestines which will help in softening of stools and ease bowel movement. Supplemental fiber available in the marketincludes psyllium husk, methylcellulose, and calcium polycarbophil. Other recommendations include increased intake of liquids and having stool softeners. Drinking enough water and liquids will prevent stool from hardening.
  • Over-the-counter topical treatment: There are various products available to treat hemorrhoids, such as gels, hydrocortisone creams, ointments, foams, pads, and suppositories.
  • Home remedies: Soaking anus in a warm sitz bath might also help in hemorrhoids for about 10 to 15 minutes per day. One should practice good hygiene and clean anus with warm water while showering. Avoid using soap and dry toilet paper. One can also use a cold compress on the anus.

Minimally Invasive Procedure for Hemorrhoids (MIPH)

Out of the various types of medical and surgical interventions available to treat Piles, Minimally Invasive Procedure for Hemorrhoids (MIPH) 0r painless stapler surgery is known to be very popular since it acts an alternative approach to surgical interventions made for piles.

MIPH is defined as a minimally invasive surgical technique in which a band of loose or prolapsed mucosa and sub mucosa in the rectum is excised, proximal to the piles and disrupted mucosa is fixed by ‘stapled end to end mucosal anastomosis.

This will ultimately occlude the blood supply to the superior hemorrhoidal artery above the hemorrhoidal tissue, resulting in the retraction of prolapsed mucosa back to its original anatomical position.
