For d 3 & b 12, visit physician for the weekly supplement. Apart from this take morning sunlight for abt 20 - 30 min everyday. Take milk & milk products. Take suncharged water too. I noted down your history. Here are the few points I would like to add from my side.
1) your pulse rate was/is high because your hb was/is low. 2) now as it is better than before but still you should take some supplements for vitamin b12 as well. 3) consume diet rich in iron like spinach, beetroot, dryfruits, eggs etc.
4) if all the above is corrected and still pulse is high then need to see for other issues like anxiety.

Yes it could be better go for a physical check-up to conclude things
in the mean time take vit-a capsules -pills once daily

Folate should ideally be started from 3 months before conception. Doses may vary according to your previous history of pregnancy. And iron tablet are taken from 2nd trimester onwards if your hemoglobin level are within normal range.
