I think you need further evaluation with the help of 2nd opinion from another proctologist nearby. If nothing is there, get it double checked. U might need an mri peri anal area to see the thing happening there. Till then, you continue with hot sitz bath with betadine, antibiotics and laxatives if required. I hope it will be helpful.

as you are pregnant now, you need to rely upon medicines primarily and to avoid any kind of unnecessary surgery. U may continue applying the ointment and sitting in warm water with betadine liquid in it.
