I think you need further evaluation with the help of 2nd opinion from another proctologist nearby. If nothing is there, get it double checked. U might need an mri peri anal area to see the thing happening there. Till then, you continue with hot sitz bath with betadine, antibiotics and laxatives if required. I hope it will be helpful.

as you are pregnant now, you need to rely upon medicines primarily and to avoid any kind of unnecessary surgery. U may continue applying the ointment and sitting in warm water with betadine liquid in it. After delivery only you may be re evaluated and think about any re do surgery if at all it is absolutely necessary. I hope it will be helpful.
