Ok according to you provided history you don't upploaded the swelling on your buttock to look it's fine see any kind of swellings in the all over body.
Not much harmful unless it's causing daily activities gets difficult if it's hurting much or causing pain you should go for smal excision by local anestesia it want cause any problem means take clavam as your doctor said but it's chronic then final step it's surgery only as I said it's on final stage.

Unfortunately, pilonidal cysts do come back after surgery. Studies show recurrence rates are as high as 30 percent. The cysts may return because the area gets infected again or hair grows near the incision scar. People who have recurrent pilonidal cysts often develop chronic wounds and draining sinuses

No possibility of any leakage at affected site after operation, I am do ksharsutra procedure since long time but no leakage observed after ksharsutra procedure. Better to consult in clinic after examination I will suggest best for you.

A pilonidal cyst is an abscess or boil. Treatment may include antibiotics, hot compresses and topical treatment with depilatory creams. In more severe cases it needs to be drained, or lanced, to heal.
