Piles or Haemorrhoids is a disorder characterized by bleeding from anal opening during defecation. This bleeding can be in the form of drops or spurts. If untreated; mass protruding out from rectum is noticed. Initially this Pile mass gets reduced by itself but later on it progresses further so manual reduction of Pile mass becomes necessary. Later on these pile masses come out even on coughing sneezing, walking etc. Depending of their site of origin; these are divided in to internal and external Piles. 

How can you tell if you have Piles? 
Painless bleeding is the first sign of Piles. If it is not treated at this stage Pile mass protrudes out from Anal opening. The symptoms of piles are – 

Internal Piles: 

  1. In first stage, painless bleeding is the only symptom of Piles. 
  2. In second stage, pile mass protrudes out from Anus during defecation which gets reduced by itself. In late second stage; Pile mass needs to be reduced manually. 
  3. In third stage; Pile masses again come out after manual reduction. During sneezing coughing and walking also pile mass come out. 

External piles: A solid/ semi-solid lump in the anal region which becomes prominent while passing stools is the external haemorrhoid. When inflamed it constitutes of coagulated blood these are known as external haemorrhoids. 

Why do Piles occur?

The Ayurvedic Approach To Piles Treatment
The ayurvedic approach to treating piles is a mix of exercise, lifestyle changes, diet, supplements and medication. The primary cause of piles being constipation, a large portion of the treatment is focused on improving the patient’s digestive health and bowel movement. 

  1. Haritaki churna is recommended to soften stool. 
  2. Fibre rich foods like green leafy vegetables salads should be consumed in adequate quantity. 
  3. To soften the stools, foods like papaya, amalaki, and figs are advised. 

A regular ayurvedic colon cleanse is very beneficial to keep the digestive system in good condition. Some other measures you should adopt are – 

  1. Take plenty of liquids like water, butter milk fruit juices etc. Butter milk is specially beneficial in Piles. 
  2. Do regular Exercise 
  3. Avoid sedentary lifestyle.

  • Hemorrhoidectomy: This is the surgery done to remove piles. The physician performs a few minute cuts around the anus to remove them away. The area which is under operation is put under local or general anaesthesia. Hemorrhoidectomy is often a procedure for the outpatient case, and you are typically discharged on the same day itself. The patient usually takes about two to three weeks, to recover fully but then it can even take as long as five to six weeks to get back to normal pain-free life again.
     
  • PPHAnother surgical method is PPH. It is also known as stapled hemorrhoidectomy. In this surgery, the physician uses a device which looks like a stapler to reposition the pile and struck off their supply of blood so that they eventually shrink and die out. This surgical procedure is less painful than traditional hemorrhoidectomy as it removes haemorrhoid to where there are fewer nerve endings, so it hurts less. There will be faster recovery, few or no complications, minor itching, and bleeding.

Piles - How To Tackle Them?

Piles or haemorrhoids are swollen veins in the rectum or anus. They can have a lot of possible causes, but the exact reason is still unknown. The one most common reason that researchers cite is the excessive straining during bowel movement and increased pressure on the veins during pregnancy. Some of the common signs of this condition include discomfort, bleeding, and itching. A clot can be witnessed occasionally. There could be swelling around the anus as well.

How is a pile diagnosed?
Doctors often recognize this condition just by doing a physical exam of the rectum. By lubricating the glove and inserting a finger into the rectum, a doctor can gauge any unusual growth around the rectum area. A doctor might also use a sigmoidoscopy or a proctoscopy to witness the lower portion of the colon. Apart from this, a doctor might prescribe a colonoscopy, if he fathoms that there is a possibility of colorectal cancer or a disease related to the digestive system.

Treatment of piles

  1. Medication: In case of mild discomfort, a doctor might prescribe over-the-counter pads, ointments, creams or suppositories. Products such as these contain lidocaine, witch hazel or hydrocortisone that can relieve a person from itching and pain. One should stop consuming over-the-counter medicine, if there is no change in the condition.
  2. Haemorrhoid thrombectomy: If a clot has appeared along with a pile in the rectum, a doctor can remove it by making an incision and draining the clot. This can fetch instant relief. The best time to do this is within 72 hours of clot formation.
  3. Minimally invasive procedures: 
    • Rubber band litigation: This form of surgery involves placing a couple of rubber band at the base of the piles to cut off blood circulation. In due course, the haemorrhoids fall off. Although this form of treatment works for many people, the patient has the risk of bleeding within a few days of placing the rubber band.
    • Sclerotherapy: This form of treatment involves injecting a chemical into the haemorrhoid. The chemical shrinks the piles within a few days. There is hardly any pain involved with sclerotherapy.
  4. Surgical procedure:
    • Haemorrhoidectomy: This is the process by which a surgeon eradicates the excess tissue that might be causing the piles. The patient is sedated before performing this procedure. Some amount of pain is experienced after the procedure is over. The temporary difficulty of urination can be faced by a patient after this procedure.
    • Stapled hemorrhoidopexy: This surgery is only performed for internal haemorrhoids. The process involves blocking the blood supply. The process of stapling has less pain compared to a Haemorrhoidectomy. On the flip side, studies have shown that this procedure has an increased chance of recurrence of haemorrhoids.
