Are you feeling an excruciating pain in the anal region? Is there a lump around the area that you can feel? Commonly called Piles, hemorrhoids is a phenomenon where the veins are swollen in the anal canal. This is because the walls of the veins are pretty thin and pain or discomfort arises when there is too much pressure on the walls. This pressure occurs most commonly due to constipation or passage of hard stool. 

There are two kinds of Piles- internal piles and the external piles; the discomfort and severity depend on the kind of piles you are suffering from. 

Causes behind it: 

  1. Constipation, i.e hard stool is the most common cause for piles. This can occur when you eat food with less fiber like junk food, breads and non-veg. Also low water intake can cause constipation. 
  2. Sitting for long hours can also cause piles. 
  3. Piles can also happen in the third trimester of pregnancy

The common symptoms associated with the condition are: 

  1. Bleeding from the anus 
  2. Painful lump or swelling near the anus 
  3. Anal itching 
  4. Tenderness or pain during bowel movements

Treatment options for piles 
In few cases, the Hemorrhoids can be treated with home remedies like a change in diet, warm sitz baths, and avoiding foods that may lead to constipation. For hemorrhoids of the first and second degree, medications, home remedies and manual methods can be used. However, when the condition persists for an extended period with severe pain or bleeding, it is best to consult a surgeon. 

The piles can be treated with: 
Injection or sclerotherapy:
Treats internal hemorrhoids 
Banding: Treatment for early stage hemorrhoids 
Surgery: Open surgery, stapled surgery or Laser 

Hemorrhoidectomy: This is the surgery done to remove piles. The physician performs a few minute cuts around the anus to remove them away. The area which is under operation is put under local or spinal anesthesia

  • Recovery: Hemorrhoidectomy is often a procedure for the outpatient case, and you are typically discharged on the same day itself. The patient usually takes about two to three weeks to recover fully. 
  • PPH: Another surgical method is PPH. It is also known as stapled hemorrhoidectomy. In this surgery, the surgeon uses a device which looks like a stapler to reposition the pile and struck off their supply of blood so that they eventually shrink and die out. This surgical procedure is less painful than traditional hemorrhoidectomy as it removes hemorrhoid to where there are fewer nerve endings, so it hurts less. There will be faster recovery, minor pain and slight bleeding. 

Laser for Piles- 
With the advent of laser treatment, piles treatment has become very easy, pain free and effective. Laser fiber is used to coagulate i.e. close the swollen blood vessels that form piles. This is done in an area with very few nerves so it is almost painless. It can be done under local or spinal anesthesia as a day care procedure. Patient can be discharged on the same day and can resume work after just 1 or 2 days.

Piles, another term for haemorrhoids, are swollen blood vessels in or around the anus and rectum. Haemorrhoid swelling occurs when there is an increase in the pressure in the small vessels. Therefore, the vein walls become stretched, thin, and irritated by passing bowel movements, and this is when piles develop.

Types of Piles: Haemorrhoids are classified into two general categories:

  1. Internal haemorrhoids: Internal haemorrhoids are located far within the rectum, and cannot be seen or felt. They are normally located between 2 and 4 centimetres above the opening of the anus. The only symptom is usually bleeding.
  2. External haemorrhoids: External haemorrhoids occur on the outside edge of the anus under the skin, where there are many pain-sensing nerves. Despite the name, external piles aren’t always seen outside of the opening of the back passage.

Symptoms of piles: Generally, the symptoms of piles are not serious. They normally resolve on their own after a few days. These symptoms can also vary depending on the size, position, and grade of the pile. So before discussing the symptoms, let’s take a look at the grades. Grades are nothing but the intensity by which piles occur:

Grade 1: These are small swelling

Grade 2: These are larger swellings

Grade 3: In this case, it hangs out from the anus during bowel movements

Grade 4: In this case, the hanging from within the anus is permanent

The most common symptom is bleeding after going to the toilet or to pass stools. A lump in and around the anus, a feeling that the bowels haven’t emptied completely, watery discharge from the anus, an itchy or sore skin around your anus and pain and discomfort during or after a bowel movement are some of the common symptoms.

Cause of piles: Anyone at any age can be affected by piles. However, they are usually more common in elderly people and during pregnancy. Modern lifestyle, excessive consumption of junk food, stress, and improper eating habits accounts for a majority of anorectal complications, including piles. Diet has a pivotal role in both causing and preventing piles. People who eat high fiber diet are less likely to get haemorrhoids, but people those who prefer a diet high in processed foods are at greater risk of piles. There can be a lot of reasons. However, some of the basic reasons are low fibre diet, prolonged sitting on toilet, indigestion, obesity, diarrhoea, long-term coughing, having a family history of piles, excessive heavy weight lifting, spinal cord injury and lack of erect posture.

Prevention of piles

The preventive measures include:

  1. Eating plenty of fibre-rich foods such as fruits, vegetables, whole grains, nuts, beans, and pulses
  2. Drinking plenty of fluids such as water helps prevent constipation
  3. Avoiding sitting in the same place for long time
  4. Staying active
  5. Simplest one is to avoid fighting the urge

Stapler Surgery For Piles 
Stapling is used to treat prolapsed hemorrhoids. A surgical staple fixes the prolapsed hemorrhoid back into place inside your rectum and cuts off the blood supply so that the tissue will shrink and be reabsorbed. Piles, a condition wherein the veins in the anal canal are swollen and obstruct the normal function of defecation, starts to bleed and cause pain prolapse or produces a discharge. There are two types in it

  1. Internal: Seen inside rectum; generally does not show any symptoms, but sometime there may be protrusion causing bleeding.

Piles also known as haemorrhoids. It is a condition in which there is swelling and inflammation around the anus and in the lower rectum. It is a condition in which the person experiences extreme pain while pooping and there may also be blood in few cases. It is very common, both in men and women. There are as many as four types of piles, such as internal, external, protruding and blind piles. There are several factors, which cause piles such as:

Piles, originally are dilated vessels inside the anus. They usually do not cause pain, but mainly bleed. Other than 1st degree, other piles may be felt outside while straining at stool. Sometimes they need to be put inside with finger. Other than bleeding, there may be itching, discharge or burning. Only first degree piles need to be managed at home by fibre diet, plenty of fluids, mild exercise & avoid straing, avoid jeans type clothes & avoid sedentary life. Rest higher degrees need to managed by an expert proctologist or surgeon. Female/ladies who develop piles during pregnancy, may need to wait upto 3 months post delivery, as these may subside in usual cases. Otherwise need some treatment after that.

Many women experience a temporary encounter with hemorrhoids when they are pregnant. Quite surprisingly, most people tend to experience this in their daily lives. By the age 50, a majority of people have encountered one or more symptoms of this ailment, including rectal pain, bleeding, itching and maybe prolapse where the hemorrhoids protrude out of the anal canal. Though this ailment can be rarely detrimental, it can be a painful intrusion. But thankfully, there are a lot of things we can do about them.


How may this be diagnosed?
Hemorrhoids are usually diagnosed from a plain medical history and health exam. External hemorrhoids, more commonly known as piles, are normally apparent, particularly if a blood clot is formed. Then the physician may also inspect the anal tube with the aid of an anoscope, which is a short plastic channel interjected into the rectum with illumination. If he finds any evidence of microscopic blood or rectal bleeding in the stool, then there may be a need to perform a colonoscopy or flexible sigmoidoscopy to determine the probable causes of bleeding like cancer or colorectal polyps, specifically in women aged over 50.

How can they be managed at home?

Most of the common symptoms of piles can improve drastically with simple in-home measures. If you are suffering from occasional flare-ups, you may try the following:

  1. Eat a lot of fibre: It is recommended to add fibre to the diet; may be a fibre supplement like Citrucel, Fibre con, etc. When you drink a lot of water along with loads of fibre, stools get softened making them effortless to pass, which will reduce the pressure on the swollen veins. Eating broccoli, wheat, oats, beans, fresh fruits every day will help in decreasing bleeding caused due to piles. It also reduces swelling and inflammation. If you feel digestive issues with a sudden splurge of fibre in your diet, you may start slowly and gradually increase the amounts.
  2. Take a sitz bath: A sitz bath is a lukewarm water bath for the lower back portion of the body, which can help in relieving irritation, itching as well as spasms occurring in the sphincter muscle. You can either get small plastic tubs for a sitz bath or sit in a normal bathtub. Most physicians suggest a half an hour sitz bath after every bowel movement. Then tenderly dry the area and avoid wiping it hard.

There are several invasive treatments that can be opted if these conservative treatments are not able to help your condition. In such an instance, it is always wise to seek the advice of a medical practitioner.

Piles or haemorrhoids is a condition in which the rectal veins become varicosed, thus causing bleeding and swelling or protrusion of mass through anus. Hemorrhoids are clumps or cushions of tissue which get aggregated around the anal canal and are full of blood vessels, muscles and elastic fibres. Piles can be of various sizes and are found inside the anus or around the anal region. However, piles found inside the anal canal are way more common than the external ones and are known as Internal Piles/Hemorrhoids. In most cases, this condition can be treated by taking required medicines along with following a high-fibre diet. However, in some cases, the piles might be severe and need to be removed surgically.

The common symptoms of piles are blood discharge while eliminating faeces and abnormal enlargements around the anal region. Piles might also be characterised by excessive itching of the anal region and pain during defecating. In Ayurvedic terms, piles are called Arshas which translates to extremely painful conditions akin to the piercing of needles. This condition can be gotten rid of by availing of various Ayurvedic remedies.


Treatment of piles using Ayurveda usually includes dietary and lifestyle changes; making most of the remedies easily implementable.

Natural food softeners such as ‘jambul’, ‘amalaki’, dried figs and papaya are also recommended. Vegetables having larger amounts of roughage, such as turnip, onions, radish, ginger and bitter gourd should be added to the diet. 

Lifestyle Changes:

Some of the basic lifestyle changes include: Regular cleansing of the colon. Regular exercise is mandatory. A normal and healthy sex life should be followed. Aggravation of symptoms of piles is caused due to abnormal or excess sex. Avoiding sitting for long periods. Consume enough water in a day (about 2-3 liters). 

Surgical treatments:

In third and fourth grade piles when medicines don't work, surgery is advisable and kshar sutra treatment is an Ayurvedic surgical procedure that can remove piles masses permanently. Kshar sutra treatment must be taken by qualified and experienced Ayurveda Doctor. Piles or Haemorrhoids is a disorder characterized by bleeding from anal opening during defecation. This bleeding can be in the form of drops or spurts. If untreated; mass protruding out from rectum is noticed. Initially this Pile mass gets reduced by itself but later on it progresses further so manual reduction of Pile mass becomes necessary. Later on these pile masses come out even on coughing sneezing, walking etc. Depending of their site of origin; these are divided in to internal and external Piles. 

How can you tell if you have Piles? 
Painless bleeding is the first sign of Piles. If it is not treated at this stage Pile mass protrudes out from Anal opening. The symptoms of piles are – 

Internal Piles: 

  1. In first stage, painless bleeding is the only symptom of Piles. 
  2. In second stage, pile mass protrudes out from Anus during defecation which gets reduced by itself. In late second stage; Pile mass needs to be reduced manually. 
  3. In third stage; Pile masses again come out after manual reduction. During sneezing coughing and walking also pile mass come out. 

External piles: A solid/ semi-solid lump in the anal region which becomes prominent while passing stools is the external haemorrhoid. When inflamed it constitutes of coagulated blood these are known as external haemorrhoids. 

Why do Piles occur? 
Piles occur when pressure in the lower rectum increases unduly. Due to this undue pressure, the blood vessels in the anus and rectum become swollen and bulge out. The causes behind this undue increase in pressure are – 

  1. Chronic diarrhoea 
  2. Chronic constipation 
  3. Pregnancy 
  4. Lifting heavy weights 
  5. Putting too much pressure while passing stool 

The Ayurvedic Approach To Piles Treatment

The ayurvedic approach to treating piles is a mix of exercise, lifestyle changes, diet, supplements and medication. The primary cause of piles being constipation, a large portion of the treatment is focused on improving the patient’s digestive health and bowel movement. 

  1. Haritaki churna is recommended to soften stool. 
  2. Fibre rich foods like green leafy vegetables salads should be consumed in adequate quantity. 
  3. To soften the stools, foods like papaya, amalaki, and figs are advised. 

A regular ayurvedic colon cleanse is very beneficial to keep the digestive system in good condition. Some other measures you should adopt are – 

  1. Take plenty of liquids like water, butter milk fruit juices etc. Butter milk is specially beneficial in Piles. 
  2. Do regular Exercise 
  3. Avoid sedentary lifestyle. 

Along with this Kshar Karma is done at week or 10 days interval. In this treatment specialized Ayurvedic medicine is applied on Pile masses. Kshar Karma stops bleeding and suppresses Pile masses.
