There must be some digestion related trouble behind this. First of all it should be treated. You should have clear motion daily. Also excess intake of pungent, dry, cold and light food, repeated intake of frequent meals (when previous food is not properly digested), spicy food, alcohol, prolonged fasting, excess of exercise, weight lifting, grief, hot climate, oily and fried food, day sleep, horse or camel riding, two wheeler riding etc are the reasons behind it. Hi if pain in Anal region is not because of piles or fissure and it has been confirmed by the surgeon ,then I think you should consult a neurophysician ,who would examine you in detail and would like to find cause of your pain which is also radiating to your knees and legs .they might do a pelvic MRI to further confirm the diagnosis.
