Your penis is tilted to left and foreskin does not move back. Minor tilt in the penis is normal. I advise you to do circumcision if there is unbearable pain in moving back foreskin when the penis is erected. It is a minor operation done under local anesthesia so there is no pain. You can get it done from a good government hospital. You have to go to surgery opd in the hospital.

Paraphimosis is an uncommon medical condition in which the foreskin of penis becomes trapped behind the glans penis, and cannot be reduced (pulled back to its normal flaccid position covering the glans).if the foreskin is left retracted for a long period, some of the foreskin tissue may become edematous (swollen with fluid), which makes subsequent reduction of the foreskin difficult. Hello. When the foreskin gets stuck in the back side of penis, it causes paraphimosis. Its a medical emergency, because it will cause loss of blood supply to frontal side of penis, may cause to permanent damage of the tissues. Regarding cost of treatment it can vary from place to place.

Paraphimosis is an emergency and reduction of paraphimosis should be done immediately otherwise it will develop a swellng / oedema very soon. The reduction should done preferably by a healthcare professional. After reduction, when swelling subsides Surgery should be carried out. This is routinely called Circumscision.
