It is normal to have papules and do not require treatment. Pearly penile papules do not cause any other symptoms to develop with them.
Once a man has developed pearly penile papules, they typically remain for life. The growths can fade with age, but they do not tend to change shape, colour, or spread further over time.

pearly penile papules are not harmful or contagious and should not be a cause for concern. They are not related to sexual activity, but it is possible to mistake them for conditions with similar-looking symptoms, such as genital warts.

Pearly penile papules are asymptomatic and persist throughout life, although lesions gradually may become less noticeable with advancing age.
It wont require any treatment unless it disturbs you in your sex life or troubles in any means.

No need to worry about. It can remain there for life and even further fade out .these are papilloma. They are not harmful. If there is further change in size, or starts itching or painful then consult. Otherwise no issues. Don't worry.

Cause of pain is possible infection and inflammation. While doing sex, friction is not tolerable. For diabetics, infection should be prevented at any cost because healing takes much more time. Control diabetes and infection. Pain will go away as papules subside.

There is no home remedy for pearly penile papules. You can consult surgeon for this. Only treatment is carbon dioxide laser ablation. PPP in not any disease or STD and it has nothing to do with the cleanliness.
