To begin, close the right nostril with your thumb and inhale through the left nostril. Close both nostrils and hold your breath for a few seconds. Then, release through the left nostril. Repeat this process, starting from the left nostril.

Rotator Cuff Tears - Causes, Symptoms & Treatment Options!

Rotator cuff is the name given to four muscles and their tendons, which keep your arms in your shoulder sockets, allowing you to lift your arms and do day-to-day stuff like combing your hair, cutting vegetables etc.

The shoulder joint is a very important joint of the body and this is why any tear that happens in the rotator cuff can be devastating.

Causes

Tears can happen in rotator cuff due to a number of factors like injury and degeneration due to age. Tears that happen due to an injury to the shoulder are called acute. Degenerative tears are tears that happen due to degeneration of the tendon over time.

Common causes of degeneration are lack of proper blood circulation, which impedes the rotator cuff’s ability to repair itself. Repetitive stress is another cause where we stress the tendon by repeating the same shoulder motions again and again while playing games like tennis, cricket etc. and doing jobs that require us to extend our arms upwards a lot. Lifting heavy objects can also lead to tears.

Symptoms

Chronic shoulder and arm pain are good reasons for you to see your doctor. If you suffer from pain in your shoulder at rest, while sleeping on the affected shoulder or pain while lifting and lowering your arm, then there is no time to wait.

The doctor will do a physical examination by moving your arm in different directions. He may follow it up with X-rays and MRI to check how big the tear is in your rotator cuff.

Treatment

Once confirmed that you indeed have a rotator cuff tear, your doctor will start the treatment. The goal of any treatment is to reduce pain and make your shoulder joint work properly again.

NON SURGICAL TREATMENT:

In minor, partial cuff tears and rotator cuff tendinosis exercises for shoulder strengthening should help.

SURGERY: 

Full thickness tears and patient not improving with conservative treatment needs arthroscopic surgery for recovery which is done with small holes around the shoulder. patient with complete tear should not delay surgery as the tendon may retract and delay may make arthroscopic surgery impossible and these patients will need major surgeries like reverse shoulder replacements .

Chest Pain - Why Does It Happen?

If you thought that chest pain occurred only due to a heart-related issue like a heart attack, you are wrong. Chest pain is one of the most common reasons that people visit the emergency services and it can be caused due to problems with your heart, lungs and digestive system. 

Chest pain can be due to either of these two main reasons:

Heart- related Issues

1. Heart attacks called myocardial infarctions are very common reasons for chest pains. During a heart attack, the blood supply that serves the heart with oxygen is cut off due to plaque build-up in heart arteries causing the death of heart muscles. 

  • Other reasons for heart attacks are blood clots or torn blood vessels. 
  • Not all people having a heart attack have symptoms. Usually, people report symptoms like chest pain, upper body pain, nausea, sweating, fatigue, and trouble breathing.
  • A heart attack is a serious medical emergency for which immediate care is needed.

2. Angina pectoris is a type of chest pain that results from reduced blood flow to the heart. Low blood flow means your heart doesn’t get enough oxygen. The angina pain is often triggered by physical activity or emotional stress and feels like a vice squeezing your chest or a heavy weight on your chest. It can also spread from your chest to your neck, arms, and shoulders. Other symptoms are fatigue, dizziness and nausea and these normally last up to 15 minutes only.

3. Unstable angina which causes crushing chest pains suddenly and even when you are not doing anything strenuous is a medical emergency as it can lead to a heart attack. This usually shows that your heart, arteries are narrowing alarmingly.

4. Other heat-related reasons for chest pains are-

  • Mitral valve narrowing or stenosis
  • Heart arrhythmias
  • Coronary Artery disease
  • Inflammation of heart muscles or pericardium and weakening of the heart muscles. 

All of these are emergencies and need immediate medical attention to prevent a fatality.

Not Heart- related:

1. Lung conditions like Pneumonia, Bronchitis, Pulmonary embolism

  • Pneumonia is an infection in one or both lungs caused by fungi, bacteria, or viruses. It causes inflammation in your lung’s air sacs and makes them full of pus which makes it difficult to breathe and causes chest pains, amongst other debilitating symptoms.
  • Bronchitis is an inflammation of your bronchial tubes. It can be chronic or acute and it can cause a cough with mucus and chest pains. 
  • Pulmonary embolism is a blood clot that occurs in the lungs. It can go on to damage parts of your lung due to limited blood flow. Apart from this, it can also decrease oxygen levels in the blood and affect other organs. 

It can be life-threatening due to permanent lung damage. The most common symptom of a pulmonary embolism is gradual or sudden shortness of breath

There is also- 

  • bluish skin tone 
  • anxiety
  • fainting
  • hest pain that extends into your arm, jaw, neck, and shoulder
  • lightheadedness

2. Rib cage inflammation or Costochondritis is also known as Tietze’s syndrome. It is an inflammation of the cartilage in the rib cage. Patients experience chest pain in the upper and middle rib area on either side of the body due to this. The pain can also radiate to the back or the abdomen, getting worse if you move, stretch, or breathe deeply.

3. Food pipe disease or Esophagitis is any inflammation or irritation of the oesophagus, a tube that sends food from your throat down to your stomach.

Common causes of food pipe disease are acid reflux or GERD and bacterial or viral infections. 

GERD- This condition occurs when the stomach contents and acids back up into the oesophagus. This causes chest pain that can mimic a heart disease. There may also be oesophagal spasms due to GERD.

The Anterior Cruciate Ligament or ACL tear is a very common problem with athletes and with people who have an active lifestyle. In fact, this can also be caused by any accident due to sudden movement.

Importance of ACL

The ACL is the supporting ligament, which runs diagonally from back to front right in the middle of the knee, connecting the femur bone and the tibia, and helps the tibia not to come before the femur. Thus the balance of the knee during back and forth movements and the entire stability of the knee joint is secured by this very important piece of ligament.

What leads to an injury to the ACL?

Here are a few actions that may result in a ligament tear.

  1. Sudden stopping while running or slowing down from a fast speed

  2. Sudden change in direction

  3. Uncontrolled jumping from a height

  4. Direct hitting with an object or person

What happens when the ACL is torn?

If the ACL is torn, the stability of your knee is gone, and you feel as if you have no control over the joint. Moreover, it causes extreme pain and instant swelling. Swelling usually appears within 24 hours of the injury.

Treatment of ACL Injury-

Recovery from an ACL tear is possible only after a surgery. Without surgery, the affected will not be able to return to sports or athletics, or may not be able to run and make swift movements again as before. However, to avoid surgery, there are non-invasive treatments for the aged and people who require moving less.

  • Non-invasive Treatment: Bracing is one treatment where braces are given to support the knee joint during movement and also to avoid further injury. This is effective for the elderly who need minimum movement and may manage daily activities with a braced knee. Also, crutches add to the support and let the weaker knee get rest. This is followed by physical therapy. But the swelling has to reduce before starting it. Many exercises and controlled movements over a long time help in getting back agility in movement gradually.

  • Surgical Treatment: Once the swelling subsides, doctors would plan for an Arthroscopic (Key-Hole Surgery) ACL reconstruction. In this Doctor make a graft of a tendon. This graft is inserted into the joint and fixed in anatomical position in femur and tibia Arthroscopically so that the newly created ACL functions like the natural one. This is the only way to get back the ligament in whole as the torn ligament cannot be stitched. Surgery is done through arthroscope and recovery time is 15 days for resuming day to day activities and 3 to 6 month for resuming sports activities with continued physical therapy after the surgery.

What Is Addison's Disease?

While cold and cough are common, diseases like Addison’s disease are rare. It is also known as chronic adrenal insufficiency or hypocortisolism as it is a result of insufficient production of hormones like cortisol and aldosterone by the adrenal glands. This disease can affect men and women of any age.

There are two types of Addison’s disease. These are:

  1. Primary adrenal insufficiency: In this case, the adrenal glands themselves have a problem.
  2. Secondary adrenal insufficiency: If the hormone production in the adrenal glands is restricted by a problem that has started somewhere else like the pituitary gland, for example, it is known as secondary adrenal insufficiency.

Primary adrenal insufficiency is more common of the two. This could be triggered as a result of autoimmune diseases or a long lasting infection such as HIV, tuberculosis and some fungal infections. Cancer can also cause this type of Addison’s disease.

Secondary adrenal insufficiency is usually caused by a problem with the pituitary gland or the hypothalamus as they are responsible for the production of hormones that stimulate the adrenal glands. Damage caused to the pituitary gland by tumors or radiation and surgery can also interfere with hormone production. Prolonged and improper use of steroid hormones can also trigger this condition.

Addison’s disease has a very slow progression and its symptoms develop gradually over time. Some of these symptoms are:

  1. Weight loss and loss of appetite
  2. Extreme fatigue
  3. Hyperpigmentation
  4. Low blood pressure
  5. Craving for salt
  6. Nausea and diarrhea
  7. Pain in the abdomen and joint pains
  8. Irritability
  9. Depression
  10. Loss of body hair and sexual dysfunction in women

Treatment for Addison’s disease involves hormone replacement therapy. This aims at correcting the hormone levels in the body. Hormone replacement therapy includes oral corticosteroids and corticosteroid injections. The dosage of these steroids could vary depending on the situation. For example, the doctor may suggest a temporary high dosage when the patient is in a stressful condition. Along with this, sodium is also recommended especially during the summer months and while exercising. This medication must be taken regularly as skipping a single dose can be dangerous.

In some cases this condition can become life threatening. An addisonian crisis causes a drop in blood pressure, sugar and a spike in potassium levels in the body. This requires immediate medical attention and can be treated with intravenous injections of hydrocortisone, saline solution and sugar.
