Your most of the symptoms may be due to ovarian cyst. Discharge from vagina may be due infection. Usually these cysts are benign. You tske treatment for both of these problems.

If ultrasound shows 12 mm fibroid is towards the inner lining of the uterus it can be removed by hysteroscope. If it is towards the outer surface of the uterus, it can be removed by laparoscopy. This is provided there is only one fibroid. Hysterectomy will not be necessary and ovaries can be conserved if you don't have family history of uterus, ovary or breast cancer.

I would suggest repeating a urine for routine and culture again. This seems like a urine infection. If it still comes out negative an empirical course of antibiotics for general pelvic infection and some vaginal medication for the discharge could be given.

What was the stage of the ovarian cancer during the treatment last year. When was the ca 125 value 10 (which date) and when was it 15. Small rise in ca 125 can be followed up. Repeat ca 125 after a month. If its showing a persistent rise, she may need further evaluation.
