Quit habits strictly. Seems like you have oral submucous fibrosis (osmf). Kindly consult an oral & maxillo facial surgeon in person for further suggestion. Treatment is surgical that can be done by experienced person only.
Medicines are helpful in great extent but cannot cure the disease completely. Antioxidants, muscle relaxants, analgesic and steroids are the drugs commonly used.
Exercises are few like blowing balloons, ice cream sticks supported stretching.

Local anesthesia has very minimal effects on patients with severe lvdysfunction. If like cataract surgery perse risk will be very low. Inspite of that surgery itself carries a bit higher risk in case of severe lvdysfunction.
