Your present condition (OSMF )is relatively progressed to a stage where mouth exercises and medicines or intra oral steroid injections will not repair or bring the situation back to normal.
Good news is its curable with surgical intervention and the best results obtained is from laser surgery.
For oral pain you can apply dologel or hiora-sg gel. For more detail you can connect with us.
THANK YOU.

Gutkha chewing habit may lead to a potentially malignant condition known as OSMF.
You have to quit the habit ASAP. Try nicotex or nicorette chewing gum. If mouth opening is reduced you have to personaly visit an expert MDS dentist in oral medicine.

Tiny bumps on the base of the tongue are taste buds or papillae. Generally, the most effective treatment for black tongue is the daily use of a tongue scraper, which removes the dead keratinized cells from the top of the tongue. Stopping or reducing any habits that might cause irritation (smoking, regular consumption of soft drinks) to the top of the tongue also usually helps reduce the problem. Of course it is important to realize that this is a harmless condition, and if it doesn’t cause too m...more

It can be due to a sharp tooth present in your oral cavity that might be causing ulcers on your tongue. I would recommend you to see your dentist for the same. They will make blunt if present. Hope this helps!
