Ulcer, bleeding, severe throat infection, recurrent infection, swelling lymph nodes of cervical region, but this may or may not be due to cancer ,u need to consult a ENT specialist.

Do a PET CT scan whole body to rule out cancer. Can do an endoscopy to see the bleeding point if PET CT is bot affordable.

First stage oral cancer is curable with good prognosis. There is less chance of recurrence. Hopefully the treatment was proper and completed. But you need regular follow up to detect recurrence earlier if it occurs. Also there is chance of second primary due to field cancerisation.
You need to quit habits like tobacco, alcohol or supari if any.

Its too early to get anxious. Oral sex once does not mean that you are infected and you have to start vaccination for it. Its better to avoid un protected oral sex in the future.

Blackish pigmentation is due to melanin deposition, which could be generalised as in smoker's melanoma or endocrine causes of same. Looks like a case of recurrent Carcinoma Tongue. The only options are Palliative chemotherapy or Immunotherapy.

I think you mean Biopsy sample. The Biopsy is taken by Oncosurgeon and sample sent to the pathology lab to confirm the cancer. The surgeon may ask for imaging like CT scan, MRI or ultrasound if he feels it to be necessary. The prognosis of advanced tongue cancer is quite poor. If need be a nasogastric tube can be inserted through nose for feeding or a PEG tube which is Inserted into stomach endoscopically. His nutrition and energy should improve with the tube feeds. Do check his electrolytes (sodium and potassium) as they may be deranged due to decreased food intake.
