the size of the breast and nipple depend on familial. Developmental and hormonal factors. Breast undergo hypertrophy along with nipple when one crosses puberty or is about to conceive. Flat nipples may be congenital. Which can lead to difficulty in breast feeding. Minor degrees get corrected with breastfeeding while the rest requires surgery.

Rough foreplay along with hormonal changes can result on sore nipples. Sexual activity can be cause of nipple pain. Body friction or sexual activity that involves the nipples can cause soreness. This pain is usually temporary and is often treated by simply giving the nipples time to heal.

Using moisturizers or nipple guards may help keep friction to a minimum and prevent symptoms from getting worse.

Yes it may be, but be sure with investigating, like, ct scan, mri, usg, etc do not leave like that if it is on 1st stage can be treated otherwise difficult to treat better to consult Dr.
